超声对比显像诊断进展期胃癌及评价

目的:探讨超声对比显像诊断进展期胃癌的价值. 方法:对临床高度疑诊63例进展期胃癌,首选超声对比显像检查,并与胃镜及手术病理诊断对照分析. 结果:超声诊断胃癌59例,其中肿块型23例,局限溃疡型8例,浸润溃疡型3例,浸润型25例. 胃镜活检证实:1例误诊,1例漏诊,诊断符合率96.6%,敏感性98.3%, 特异性75.0%. 结论:超声对比显像能确切反映进展期胃癌病理形态的切面特征,可为临床提供可靠定性及其分型诊断的依据.

Value of contrast ultrasonography in diagnosis of advanced gastric cancer

AIM: To detect the value of contrast ultrasonography in the diagnosis of advanced gastric cancer. METHODS: Sixty three patients with suspected advanced gastric cancer were examined by contrast ultrasonography after taking in gastric contrast medium that mixed by 10 mL ultrasonographic contrast medium and 400-600 mL pure water. The 63 patients also had gastroscopy and biopsy and results of contrast ultrasonography were compared with those of gastroscopy and biopsy. RESULTS: Fifty nine patients were diagnosed to have gastric cancer by contrast ultrasonography, among whom, 23 were lumpish shaped, 8 localized ulcer like, 3 infiltrative ulcer like and 25 with infiltration. Gastroscopy directed biopsy findings showed that 1 case had been misdiagnosed positively and another case had been misdiagnosed negatively. The total diagnostic coincidence, sensitivity and specificity, was 96.8%, 98.3% and 75.0%, respectively. CONCLUSION: Contrast ultrasonography can accurately reflect the morphologic characteristics of advanced gastric cancer and can also provide more reliable information for the diagnosis and classification of gastric cancer.
