慢性心力衰竭患者血清心肌肌钙蛋白I与心肌重构及心脏功能的相关性分析

目的：探讨慢性心力衰竭（CHF）患者血清心肌肌钙蛋白I（cTnI）与心肌重构、心脏功能的相关性。方法：选择110例CHF失代偿患者，根据测得的血清cTnI浓度，分为观察组和对照组，观察组50例cTnI升高，对照组60例cTnI正常。应用心脏彩色多普勒超声诊断仪测定心脏结构改变指标及心脏功能指标，分析cTnI与心脏结构、功能指标间的相关性，并对比两组结果。结果：观察组和对照组比较，左心房内径（LAd）和左心室舒张末短轴内径（LVEDd）及左心室后壁厚度（LVPW）、室间隔厚度（IVS）、左心室射血分数（LVEF）、左心室重量（LVM）、左室短轴缩短率（FS）有明显差异（P＜0．05）；观察组血清cTnI浓度与LAd、LVEDd 正相关，与LVPW、IVS、LVM、LVEF、FS负相关（ P＜0．05）；Logistic回归分析显示病程、LAd及LVEDd是导致血清cTnI浓度升高的危险因素。结论：CHF患者血清cTnI与其心肌重构和心脏功能有明显相关性，监测CHF患者的血清cTnI对治疗和预防心衰有重要的临床意义。

Correlation Analysis of CHF Patients with Serum Cardiac Troponin I between Myocardial Remodeling and Cardiac Function

Objective： To analyze the correlation of chronic heart failure （ CHF ） patients with serum cardiac troponin I （ cTnI ） between cardiac remodeling and cardiac function.Method： 110 CHF patients were divided into the observation group（50 cases with elevated cTnI） and control group（60 cases with nor-mal cTnI）.The cardiac structure change indicators and cardiac function indicators were measured , analyzed the correlation about cTnI between cardiac remodeling and cardiac function, and compared the results be-tween the two groups.Result： Compared with the control group, there were statistically significant in the LAd and LVEDd , LVPW, IVS, LVEF, LVM, FS（ P 〈0.05）;the cTnI and LAD, LVEDd were positive correlation （P〈0.05）, and with LVPW,IVS,LVM,LVEF,FSCO ,SV were negative correlation（P〈0.05）;Logistic regression analysis showed that the disease course , LAD and LVEDd were risk factors in leading to elevated serum cTnI.Conclusion：The cTnI of CHF patients significantly correlates with cardiac remodeling and cardiac function , It＇s important to monitor the cTnI of CHF on the treatment and prevention of heart fail-ure.
