血清脂联素水平与急性脑梗死患者颈动脉粥样硬化斑块关系的研究

摘    要:目的探讨急性脑梗死患者颈动脉粥样硬化斑块稳定性与血清脂联素水平的关系。方法选择113例首次发病的急性脑梗死患者为病例组和31例健康对照者为对照组,采用多普勒超声检测颈动脉内膜中层厚度及斑块。根据斑块的性质,将病例组分为无斑块组39例、稳定斑块组34例和不稳定斑块组40例。采取ELISA法检测脂联素水平并分析。结果病例组脂联素水平低于对照组(P<0.01)。病例组颈动脉硬化发生率、颈动脉斑块发生率、颈动脉狭窄发生率均高于对照组(P<0.05)。不稳定斑块组脂联素水平低于无斑块组、稳定斑块组和对照组(P<0.05)。稳定斑块组脂联素水平低于无斑块组和对照组(P<0.05)。无斑块组脂联素水平低于对照组(P<0.05)。病例组入院第14天脂联素水平仍高于入院48 h(P<0.05)。结论急性脑梗死患者脂联素的降低与颈动脉粥样硬化斑块的存在及性质显著相关。

Relationship between serum adopinectin levels and the carotid plaque in patients with acute cerebral infraction
Abstract:Objectives To explore the relationship between stability of carotid plaque and serum adopinectin(APN) in patients with acute cerebral infarction(ACI).Methods 113 patients with initial acute cerebral infarction and 31 healthy persons were checked by using carotid ultrasonographic methods.Patients were divided into three groups:no carotid plaques(NCP,n= 39),stable plaques(SP,n = 34),vulnerable plaques(VP,n = 40).The serum concentration of APN was measured by means of ELISA.Results The serum level of APN was significantly lower in the patients with ACI than that in the healthy controls(P<0.01).Incidence of carotid artery athermatosis, plaque and stenosis were higher than healthy controls(P<0.05).The serum level of APN was significantly lower in VP than that in NCP,SP and the healthy controls(P<0.05);the serum level of APN was significantly lower in SP than that in NCP and the healthy controls(P<0.05); the serum level of APN was significantly lower in NCP than that in the healthy controls(P< 0.05).The serum APN levels in the patients at day 14 after suffering from ACI were more than that in the patients within 48 hours after the suffering from ACI(P<0.05).Conclusions The decrease of serum APN levels significantly correlated with the existence and character of carotid plaque in patients with ACI.
