卡介苗加丝裂霉素膀胱灌注预防膀胱癌术后复发的疗效

目的提高膀胱癌的治疗效果,降低术后复发率。方法74例膀胱癌患者分为两组,(1)卡介苗(BCG)加丝裂霉素C(MMC)组(35例),MMC40mg+生理盐水50ml膀胱灌注,每周1次。连续6次后改为BCG120mg+生理盐水50ml膀胱灌注,每周1次,连续6次后改为1个月1次维持至2年。(2)BCG组(39例),BCG120mg+生理盐水50ml膀胱灌注。每周1次,连续8次后改为1个月1次维持至2年。结果随访6~36个月,BCG加MMC组复发3例,复发率为8%,BCG组复发9例,复发率为23%,两者比较差异有显著性。结论BCG加MMC膀胱灌注预防膀胱癌术后复发疗效优于单用BCG膀胱灌注。

A analysis for intravesical instillation of BCG combined with MMC to prevent the recurrence of bladder cancer after surgical management

Objective To improve curative effect of bladder cancer.Methods A total of 74cases of bladder cancer were divided into 2 groups.(1) Group1 received intravesical instillation of BCG combined with MMC (35 cases).Intravesical instillation of MMC 40mg and physiological saline solution 50ml.Every week only once,BCG 120mg and physiological saline solution 50ml instead after 6.Every week only once,every month only once, instead after 6 and keep on 2 years.(2) Group 2received BCG alone (39caese).Intravesical instillation of BCG 120mg and physiological saline solution 50ml.Every week only once,every month only once instead after 8 and keep on 2 years.Result The patients were followed up for 6-36 months.In group of BCG combined with MMC 3 caese had recurrence,the incidence rate being8%.In group of BCG alone 9 cases had recurrence,the incidence rate being 23%.The recurrnce rate was significantly different between the 2 groups.Conclusion The curative effect of intravesical instillation of BCG combined with MMC to prevent the recurrence of bladder cancer after surgical management is higher than that of BCG therapy.
