综合康复训练联合酸刺激对脑卒中后吞咽障碍患者吞咽功能及营养状况的改善作用

目的：探讨综合康复训练联合酸刺激对脑卒中后吞咽障碍患者吞咽功能及营养状况的改善作用。方法：选择脑卒中合并吞咽障碍患者80例,随机分为实验组和对照组,各40例,对照组采用运动训练及吞咽功能障碍训练治疗,实验组采用综合康复训练联合酸刺激法治疗,比较两组患者治疗前、治疗4周时吞咽功能、营养状况。结果：治疗4周时,实验组患者Burk评分明显低于对照组（P〈0.05）,VFSS评分明显高于对照组（P〈0.05）。治疗后两组患者血ALB、TG、TC、BMI、TSF与组内治疗前比较有明显升高（均P〈0.05）,且实验组治疗后血ALB、TG、TC、BMI、TSF均明显高于对照组（均P〈0.05）。结论：综合康复训练联合酸刺激能更有效地改善卒中后吞咽障碍患者吞咽功能,增加营养摄入,从而促进神经肌肉功能的恢复,提高临床疗效,值得临床推广应用。

Effect of swallow and nutrition status in patients stroke merged with dysphagia treated by comprehensive rehabilitation training combined with acid stimulation

Objective： To investigate effect of swallow and nutrition status in patients with dysphagia after stroke treated by comprehensive rehabilitation training combined with acid stimulation. Methods： 80 patients stroke merged with dysphagia were selected and randomly divided into combination group and control group. There were40 cases in each group. Control group were received exercise training and swallowing dysfunction training,Combination group were treated with comprehensive rehabilitation training combined with acids timulation. The swallowing function and nutritional status of patients before treatment or at four weeks after treatment were compared between two group. Results： At four weeks after treatment,the Burk scores of combination group was significantly lower than that of control group（P 〈 0. 05）,the VFSS score was significantly higher than that of control group（P 〈0.05）. The serum ALB,TG,TC,BMI,TSF of two groups after treatment significantly increased than before treatment（all P 〈 0. 05）,but after treatment,the serum ALB,TG,TC,BMI,TSF of combination group were significantly higher than those of control group（all P 〈 0. 05）. Conclusion： Comprehensive rehabilitation measures combined with acid stimulation training can effectively improve patients’ s swallowing function with post-stroke swallowing disorder,increase their nutritional intake,so as to promote recovery of neuromuscular function,and improve clinical efficacy. It is worthy of clinical application.
