失血性休克再灌注心肌损伤机制及灵芝多糖的预防作用

摘    要:目的 探讨失血性休克再灌注 (HS -R)过程中心肌损伤的机制及灵芝多糖 (GLP)的影响。方法 复制家兔HS -R模型 ,随机分成 3组 ,A组为假手术组 ,B组为生理盐水 (NS)再灌注组 ,C组为 1%GLP再灌注组 ,观察平均动脉血压 (MAP)、心功能、血清心肌酶及心肌超氧化物歧化酶 (SOD)活性、丙二醛 (MDA)含量的变化。结果 B、C组动物在失血性休克 4 0min时左心室功能明显降低 (P <0 .0 5 ) ,血清心肌酶显著升高 (P <0 .0 5 ) ,B组动物再灌注4 0min时 ,心功能进一步降低 (P <0 .0 5 ) ,血清心肌酶进一步升高 ,心肌SOD活性明显低于A组 ,MDA含量明显高于A组 (P <0 .0 5 ) ;C组动物再灌注 4 0min ,与B组相比 ,心功能明显改善 ,血清心肌酶、心肌MDA含量明显降低 ,心肌SOD活性明显升高 (P <0 .0 5 )。结论 HS -R过程中心肌损伤与脂质过氧化反应增强有关 ,GLP对此损伤有保护作用

Abstract:Objective To study the mechanism of myocardial injury in hemorrhagic shock reperfusion and the effects of GLP on it.Methods With the model of HS R in rabbits, animals were divided into three groups: sham operation group(A), reperfusion with NS group(B), reperfusion with 1% GLP group(C). The mean artery pressure(MAP), ventricular function, myocardium enzyme in serum, content of MDA and activity of SOD in myocardium were all examined. Results In the course of HS, ventricular functions in group B and C were reduced significantly compared with ventricular functions before shock and in group A( P < 0.05), but the myocardial enzymes in serum were increased significantly. When the animals in group B were reperfused with NS, compared with ventricular functions and nuyocardium enzymes in serum in the course of HS, ventricular functions were reduced and the myocardium enzymes in serum were increased significantly( P < 0.05). The activities of SOD in myocardium were decreased and the contents of MDA in myocardium were increased significantly compared with those in group A.When the animals in group C were reperfused with 1% GLP, the parameters all above were ameliorated.Conclusions The myocardial injury in HS R is related with lipid peroxidation. GLP has protective effect on myocardial injury in HS R.Its mechanism will enhance the activity of SOD and inhibiting lipid peroxidation in myocardium.
