摘    要:目的 评价膜式液基薄层细胞学技术(TCT)和人乳头状瘤病毒(HPV)分型技术在宫颈癌筛查中的临床应用价值.方法 选择2009年1月-2011年12月在淳安县妇幼保健院门诊就诊的3759例有性生活史的患者进行TCT检测,对其中TCT检测结果异常的287例患者采用杂交捕获技术检测HPV分型.对TCT检测和HPV检测结果均阳性的116例患者进行宫颈组织病理活检,考察两种技术在宫颈癌筛查中的应用价值.结果 3759例中,有287例经TCT检查子宫颈出现异常病变,占7.64%,其中非典型鳞状上皮细胞(ASCUS)178例,低度鳞状上皮内病变(LSIL)93例,高度鳞状上皮内病变(HSIL)12例,鳞状细胞癌和腺癌(CA)4例.287例异常病变患者中,116例经HPV检测呈阳性,阳性率为40.42%.结论 TCT检测联合HPV分型技术能进一步提高宫颈癌的检出率.

Abstract:OBJECTIVE To evaluate the clinical value of thinprep cytologic test (TCT) together human papilloma virus (HPV) genotyping in cervical cancer screening. METHODS Clinical data of 287 abnormal outpatient patients in 3 759 cases who had sexual life history and TCT detection from January 2009 to December 2011 in Chun'an County Maternal and Child Health Hospital were retrospectively analyzed. And HPV type were detected by using the hybrid capture technique. TCT detection and HPV test results were positive in 116 patients with cervical tissue biopsy to study the two technologies in cervical cancer screening. RESULT 287 cases of TCT examination were abnormal, accounting for 7.64%. And 178 cases of atypical squamous cells(ASCUS), low-grade squamous intraepithelial lesions (LSIL) in 93 cases, a high degree of squamous intraepithelial lesions(HSIL)in 12 cases, 4 cases of squamous cell carcinoma and adenocarcinoma(CA). 116 cases of HPV testing were positive from 287 cases of abnormal lesions, accounting for 40.42%. CONCLUSIONS TCT detection together with HPV typing techniques can improve the detection rate of cervical cancer.
