单髁置换术治疗膝关节自发性骨坏死的近期临床疗效分析

目的探索单髁置换术治疗膝关节自发性骨坏死的近期临床疗效。方法2017年2月至2019年5月,采用单髁置换术治疗30例膝关节自发性骨坏死患者。术后采用VAS评分、HSS评分、膝关节活动度、胫股角等指标对疗效进行评价。结果所有患者术后随访5~9个月(平均6.5个月)。切口均一期愈合,未出现切口感染、下肢深静脉血栓、假体周围骨折等并发症。患者术后膝关节疼痛减轻,功能改善明显,HSS评分、VAS评分、胫股角等指标均较术前有明显改善(P<0.05),而关节活动度手术前后无明显差异(P>0.05)。结论单髁置换术治疗膝关节自发性骨坏死的近期临床疗效满意。

Short-Term Clinical Effectiveness of Monondylar Replacement in the Treatment of Spontaneous Osteonecrosis of Knee Joint

Objective To evaluate the short-term clinical effect of Monondylar replacement in the treatment of spontaneous osteonecrosis of knee joint.Methods From February 2017 to May 2019,30 patients with spontaneous osteonecrosis of the knee were treated by monondylar replacement.VAS score,HSS score,knee range of motion and tibiofemoral angle were used to evaluate the effect.Results All 30 patients were followed up for 5 to 9 months,with an average of 6.5 months.All incisions healed by first intension,and no postoperative complications such as wound infection,deep vein thrombosis of lower extremity,and fracture around prosthesis occurred.The pain of knee joint was alleviated,the function was improved.HSS score,VAS score and tibiofemoral angle were significantly improved compared with the preoperative results(P<0.05),and there was no significant difference between preoperative and postoperative range of motion(P>0.05).Conclusion Monondylar replacement for the treatment of spontaneous osteonecrosis of the knee is satisfactory.
