腰椎间盘突出合并侧隐窝狭窄症

82例腰椎间盘突出合并侧隐窝狭窄症,占同期腰椎间盘突出手术总数的47.4％。男49例,女33例。年龄28～70岁,平均46.3岁。9例全椎板切除,35例半椎板切除,38例椎板间开窗手术。术后73例经1～5年随访,优46例,良23例,进步4例。优良率达94.5％。本病的临床表现往往是一组混合症状,确诊需要靠CT扫描检查。治疗以手术为主,手术进路以半椎板切除、椎板间开窗为首选。手术的关键不但要彻底摘除突出的髓核组织,而且要使神经根完全减压,并强调术后的稳定治疗。这对于提高治愈率和远期疗效有重要的临床价值。

Protrusion of Lumbar Intervertebral Disc Complicated with Lateral Recess Stenosis

The Paper reported 49 male and 33 female cases of protrusion of lumbar in-tervertebral disc complicated with lateral recess stenosis , which accounted for 47. 4% of all the patients suffering from protruted lumbar intervertebral disc , treated by opetation. The age range was from 28 to 70 years old with an average of 46. 3 years. 9 cases underwent total laminectomy ,35 hemilaminectomy and 38 laminal fenestration. One year to 5 years follow up of 73 cases showed 46 cases were excellent ,23 cases good and 4 cases improved . The rate of the good and excellent was as high as 94. 5%.Clinically this disease presents a severe ,miscellaneous group of symptoms. Confirmative diagnosis depends on CT scanning. The main procedures are hemilaminectomy and laminal fenestration. The successful operation is made by thorough removal of protrusion of lumbar intervertebral disc and complete decompression the nerve root ,postoperative stable treatment being emphasized. These are very important in the cure rate and late result.
