两针法股静脉穿刺置管抢救小儿失血性休克

目的为失血性休克患儿快速建立有效补液途径,提高抢救成功率。方法将100例外伤致失血性休克患儿随机分为观察组和对照组各50例。对照组采用传统股静脉穿刺方法;观察组以股动脉搏动内侧0.3～0.5cm或脐部引一直线垂直于腹股沟的垂直交叉点内侧约0.5cm定位为第1针穿刺点,第1针穿刺点垂直下方3.0～4.0cm处为第2针穿刺点留置股静脉置管。结果观察组留置成功率显著高于对照组,操作时间显著短于对照组(均P<0.01)。结论两针法股静脉穿刺置管能保证失血性休克患儿静脉通道及时建立。

Femoral vein catheterization with two-step venepuncture for children with hemorrhagic shock

Objective To establish an intravenous line for quick fluid administration for children with hemorrhagic shock,and to improve success rate of resuscitation.Methods One hundred children with hemorrhagic shock were assigned to two groups of 50.The control group received traditional femoral vein catheterization,while participants in the observation group were subjected to femoral vein catheterization with two-step venepuncture: the first puncture point was located at 0.3-0.5 cm medial to the palpable femoral artery or was positioned approximately 0.5 cm medial to intersecting point by drawing a line from umbilicus perpendicular to inguinal canal;the second puncture point was about 3.0-4.0 cm vertically below the first puncture point for catheterization.Results The success rate of catheterization in the observation group was significantly higher than that in the control group,the operation time was significantly shortened (P<0.01 for both).Conclusion Femoral vein catheterization with two-step puncture can quickly establish an intravenous line for fluid administration for children with hemorrhagic shock.
