2型糖尿病患者血清网膜素与糖脂代谢的关系

摘    要:
目的:测定新诊断2型糖尿病(T2DM)患者血清网膜素、内脂素水平,探讨网膜素与体脂参数、糖脂代谢、胰岛素抵抗及内脂素的相关性。方法:共选取研究对象73名,其中包括健康对照组28例和新诊断的T2DM患者45例,采用ELISA法检测空腹血清网膜素-1与内脂素水平,同时检测空腹血糖(FPG)、口服葡萄糖耐量试验2h血糖(OGTT-2hPG)、糖化血红蛋白(HbA1c)、血脂等各项指标及空腹胰岛素(FINS)水平,计算体重指数(BMI)和腰臀比值(WHR),并以稳态模型(HOMA-model)计算胰岛素抵抗指数(HOMA-IR),并分析网膜素-1与各指标之间的关系。结果:与健康对照组相比,T2DM组血清网膜素-1水平明显降低[(68.6±11.27)μg/L比(29.8±5.19)μg/L,P<0.05],内脂素水平明显升高[(9.37±0.85)μg/L比(18.67±7.98)μg/L,P<0.05],血清网膜素-1水平与WHR、HOMA-IR、甘油三酯、2hPG、内脂素均呈负相关(r分别为-0.310,-0.290,-0.460,-0.376,-0.040,P<0.05)。结论:网膜素-1水平的变化可能在糖尿病的发生发展中起一定作用。

Abstract:
Objective: To assay the serum omentin and vifastin level in newly diagnosed type 2 diabetes mellitus(T2DM) patients and investigate the relationship between serum omentin level and body fat parameters,glycolipid metabolism and visfatin levels.Methods: The fasting serum levels of omentin and vifatin were detected by ELISA in 45 T2DM patients and 28 healthy controls.Meanwhile,data were also abstained including blood glucose,HbA1c,lipids,insulin levels,body mass index(BMI),waist hip ratio(WHR) and insulin resistance index(HOMA-IR).The relationship of serum omentin level and these above indictors was analyzed.Results: Compared with control group,the serum omentin level was significantly lower and visfatin level was significantly higher in T2DM group as(68.6±11.27) μg /L vs(29.8±5.19) μg /L and(9.37±0.85) μg/L vs(18.67±7.98) μg/L(both P<0.05).Partial correlation analysis showed that serum omentin level was negatively correlated to WHR,HOMA-IR,triglycerides(TG),2 hour OGTT glucose(2hPG),and visfatin(r=-0.310,-0.290,-0.460,-0.376,and-0.040 respectively,P<0.05).Conclusion: The change of serum omentin level may be involved in the pathogenesis of diabetes mellitus.
