高血压病患者C反应蛋白变化的临床意义

摘    要:目的:观察高血压病患者血清C反应蛋白(CRP)的变化,分析其与相关临床指标的关系,探讨炎症在高血压病中的作用.方法:测定143例高血压病患者和60例健康体检者的血清CRP、空腹血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、白细胞(WBC)计数、体质量指数(BMI),所有研究对象均清晨空腹采血,CRP的测定采用免疫散射比浊法 .结果:高血压病组CRP水平显著高于正常对照组(P<0.01);随着血压水平的升高,CRP也逐渐升高,高血压1级与3级间有显著性差异(P<0.05);高血压各亚组WBC计数均较对照组高;CRP与TG呈显著正相关,与HDL-C呈显著负相关(P<0.05).结论:高血压病患者体内存在炎症反应; CRP水平可相应反映了高血压病患者血压水平,CRP对于病情的估计具有一定的意义;高TG、低HDL-C血症可能参与了体内炎症反应,积极控制血脂可能减轻炎症反应,从而有助于改善血压情况.

Abstract:Objective:To observe the changes of serum C-reactive protein (CRP) in patients with essential hypertension(HBP) and to analyze the relationship between C-reactive protein and related clinical markers so as to study the role of inflammation in hypertension.Methods: CRP was measured in 143 patients with essential hypertension and 60 normal controls by immunoturbidimetric assay; at the same time, their fasting blood glucose(FBG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),leukocyte (WBC) and body mass index(BMI) were also measured. Blood samples were taken in the morning without breakfast. Results: Serum levels of CRP were significantly higher in patients with hypertension than in normal controls (P<0.01) and the levels of CRP increased gradually with the increase of blood pressure.There was signficant difference in CRP levels between patients with grade 1 and grade 2 hypertension(P<0.05). Leukocyte count was higher in hypertensive group than in control group. CRP level was positively correlated with age,TG and FBG and negatively correlated with HDL-C(P<0.05).Conclusion: Inflammation reaction exists in hypertensive patients; the levels of CRP correspondingly reflect the degree of blood pressure. Hypertriglyceridemia and low level of HDL-C may participate in inflammation reaction, so controlling hypertriglyceridemia and increasing HDL-C may relieve inflammation in hypertensive patients.
