深低温冷冻小猪部分肝叶Clisson管道二级分支区域的实验研究

目的：了解深低温冷冻对部分肝叶的Glisson氏管道及相应肝组织的影响，为临床冷冻处理肝叶管道附近的癌肿提供实验依据。方法：14只小猪，随机分为2组，用冷冻头深低温冷冻实验组动物左肝外叶Glisson管道区域3分钟（冷冻时阻断肝门），对照组仅阻断肝门3分钟。术后观察动物血清AST、ALT、TBILI、ALB的改变，患肝的病理变化，并用彩超了解冷冻肝叶管道的情况。结果：冷冻可造成受冻部位肝实质、胆管及门静脉分支管壁的坏死，而肝动物分支不受影响，术后8周原冷冻区的肝动脉、门静脉分支的管腔仍保持通畅，但患肝叶萎缩、纤维化。动物可耐受上述冷冻。结论：机体可耐受对部分肝叶Glisson氏管道系统二级分支区域的直接深低温冷冻，冷冻可造成受冻部位肝实质的坏死，而达到外科治疗目的。

Study on Freezing the Secondary Branches Enclosed in Glisson''''s Capsule of Certain Hepatic Lobes with Deep Temperature in Pigs

Objective: To investigate the effect of freezing the secondary branches enclosed in Glisson's capsule of certain hepatic lobes in deep low temperature .Methods: 14 pigs were divided into 2 groups randomly. Treated group: the area of secondary branches of Glisson's system in the left lateral lobe were frozed by a cryoprobe in deep low temperature for 3mins, with the blockage of porta hepatitis blood flow. Control group: only with porta hepatitis blockage for 3mins. Serologic examination,Colour Dopper examination, as well as well as pathological observation were used to evaluate theis procedure postoperatively. Results:The frozen hepatic parenchyma, as well as the wall of the frozen secondary bile duct and portal vein showed necrosis; the frozen hepatic artery ramification had not obvious changes postoperatively.8 weeks after the freezing, the lumen of the hepatic artery and portal vein still maintained unobstructed, atrophy and fibroplasia occured in related hepatic lobe; and the animals recovered well.Conclusion: It showed that animals could tolerate the direct deep freezing of the area of secondary branches enclosed in Glisson's Capsule:The therapeutic effect could be attained by the necrosis of the frozen hepatic parenchyma.
