急性颈中央脊髓损伤的手术治疗

目的探讨急性颈中央脊髓损伤的效果观察。方法1999-2007年收治19例急性颈中央脊髓损伤,男13例,女6例,年龄46～68岁,平均56岁,所有病例针对不同情况选择三种不同的手术方式,其中前路手术5例,后路手术11例,前后路手术3例,随访时间1～3年,平均18个月。结果全部病例均无严重的术中术后并发症,平均ASIA运动平分由（43.4±15.5）分增至（83.5±9.5）分。结论对急性颈中央脊髓损伤的患者,术前全面分析,有针对性地选择手术方式,临床效果良好。

The srgical treatment of acute traumatic cervical central spinal cord

Objective To explore the surgical method and clinical effects of acute traumatic cervical central spinal cord. Methods 19 patients （13 males and 6 females） with acute traumatic cervical central spinal cord from 1999 - 2007. There were all cases with age 46 -68 years （mean 56 years）. All cases with acute traumatic cervical central spinal cord in which three approaches were used documented. In these cases, anterior procedure was performed in 5, posterior procedure in 11, combined anterior and posterior procedure in 3, and they were followed up forl to 3years, 18 months on average. Result No severe complications happened during and after operation. ASIA mtor score in 19 cases was increased from 43.4 ±15.5 to 83.5 ±9. 5. Conclusion The surgical treatment of actue traumatic cervical central spinal cord is ticklish and controversial. Carfully evaluation of a patient＇ s status before helpful to choose a aternative indicate to the patient which can ensure the outcome.
