重组人脑利钠肽治疗难治性心力衰竭的临床观察

目的 探讨重组人脑利钠肽(rhBNP)在治疗难治性心力衰竭(RHF)的价值及安全性。方法 30例RHF患者在标准抗心衰治疗的基础上应用rhBNP治疗,评估rhBNP治疗前后心功能、左室射血分数(LVEF)、N末端-脑钠肽前体(NT-proBNP)、尿量、动脉收缩压(SBP)、心率(HR)、肝肾功及血清钠等指标。结果 rhBNP明显改善RHF患者的心功能,有效率86.7%,显著提高患者LVEF、尿量及降低血浆NT-proBNP水平,较治疗前有统计学差异(P＜0.05),SBP、HR、肝肾功及血清钠较前无统计学差别(P＞0.05)。结论 rhBNP治疗RHF疗效显著,不良反应发生率低,值得推广。

Observation on clinical effect of recombinant human brain natriureticpeptide for the treatment of refractory heart failure

Objective To study the efficacy and safety of the recombinant human brain natriuretic peptide (rhBNP) in the treatment of refractory heart failure (RHF).Methods Thirty patients of RHF were given rhBNP for treatment based on standard anti-heart failure therapy.Before and after the treatment of rhBNP,cardiac function inclouding left ventricular ejection fraction (LVEF),the level of plasms NT - proBNP,urine volume,artery systolic blood pressure (SBP) ,heart rate (HR),liver and kidney function and serum sodium indicators were evaluated.Results The rhBNP significantly improves cardiac function in patients with RHF,effectiveness 86.7%;there were significant increase LVEF and urine volume,were significant decrease in level of NT -proBNP;there were different than before treatment (P＜0.05).But there were no different from SBP,HR,liver and kidney function and serum sodium than before treatment (P＞0.05).Conclusion rhBNP is effective in treating RHF and has the advantage of low incidence of side effect.It should be widely applied.
