脑室内灌注尿激酶治疗脑室出血的临床观察

本文报告应用尿激酶脑室内灌注治疗严重脑室内出血患者8例,并与另一组17例行单纯脑室外引流者作对比分析。所有患者均经CT检查证实为全脑室系统出血或以脑室出血为主,脑内血肿量少于10毫升者。根据GOS和脑室内血肿溶解时间判断疗效。单纯引流组17例中死亡11例,重残4例,中残和良好2例。6例生存者脑室内血肿完全溶解时间为6-23天,平均15.7天.尿激酶组8例中7例生存良好或中残,死亡一例。7例生存者脑室内血肿完全溶解时间为2-12天,平均4.8天。作者认为尿激酶脑室内灌注方法简单,安全,可明显降低严重脑室出血患者的死亡率和提高生存质量。

Clinical observation of Treating Intraventricular Hemorrhage with Intraventricular Infusion of Urokinase

Eight patients with intraventricular hemorrhage were treated with intraventricular infusion of urokinase. All cases were confirmed by CT that hemorrhage extended into all ventricular chambers or cerebral hemorrhage less than 10 ml. The therapy was started 1 to 2 days after onset of symtoms and the urokinase was infused one to two times a day, and into alternate ventricles in cases with bilateral ventricular drainage. The therapy was continude until clearance of all ventricles was confirmed by CT scan. Seven of the eight patients showed excellent or good outcome, and one patient died. The outcome of this group was compared with a retrospectively studied group of 17 patients not treated with urokinase. The results indicate that this therapy is a easy and safe method, and will greatly improve the progrosis of intraventricular hemorrhage.
