摘    要:目的评价多层螺旋CT曲面重组技术在诊断胰腺癌中的价值。方法45例临床怀疑胰腺癌患者均行多层螺旋CT三期增强扫描,并沿着胰管、胆总管进行重组。根据横断面图像及横断面结合曲面重组图像分别对是否有胰腺癌用盲法进行评估。结果45例中的27例经手术病理和临床资料证实为胰腺癌。对于发现胰腺癌,横断面图像、横断面结合曲面重组图像的敏感性分别为83.4%和92.6%,特异性分别为80.4%和92.8%。结论曲面重组图像易于显示胰腺、胆胰管的全貌,作为横断面图像的重要补充,有助于提高发现胰腺癌的敏感性和特异性。

Abstract:Objective To evaluate the utility of curved planar reformations in multi-slice spiral CT for diagnosing pancreatic carcinoma .Methods Forty-five patients with clinically suspected pancreatic carcinoma underwent triphasic-enhanced multi-slice spiral CT scans.Curved planar reformations were performed along the pancreatic duct and the common bile duct.The axial images and the axial images combined with curved planar reformations were independently assessed by two radiologists to determine whether the presence of pancreatic carcinoma.Results Among 45 patients,27 had pancreatic carcinomas proved by pathology and clinical data.For pancreatic carcinoma detection,the axial images and the images combined with curved planar reformations had an average sensitivity of 83.4%and 92.6%, and an average specificity of 80.4%and 92.8%.Conclusion The imaging of curved planar reformation easily shows the whole outline of pancreas,pancreatic duct,common bile duct,as an important complement of axial imaging,it is helpful to elevate the detective sensitivity and specificity of pancreatic carcinoma.
