芬太尼联合异丙酚对机械通气患者镇静效果的影响

摘    要:目的观察芬太尼和异丙酚联合用于机械通气的内科患者的镇静作用。方法把50例神志清楚的机械通气的内科病人随机分为两组,异丙酚联合芬太尼组和异丙酚组,每组25例。以MAAS镇静深度评分≤3分为镇静目标,记录用药前及用药后4、8、24、48h的收缩压(SBP)、舒张压(DBP),心率(HR)、呼吸频率(R)、血氧饱和度(Sa02)、肌肉活动评分法(MAAS)镇静深度评分,48h异丙酚用量、镇静药物费用,停用异丙酚后清醒时间。结果整个观察过程中,两组各时间点SBP、DBP、HR、R、Sa02比较,差异均无统计学意义(F分别=12.34、15.13、11.46、13.52、13.15,P均>0.05)。两组的SBP在镇静后,逐渐下降,Sa02逐渐升高,在镇静8h后最明显,与镇静前(Oh时)比较,差异均有统计学意义(t分别=8.22、13.11、12.53、9.12,P均

Abstract:Objective To investigate the role of propofol combined with fentanyl on the sedation in patients with mechanical ventilation.Methods Fifty patients were classified into two groups.One group was given propofol,the other one was given propofol and fentanyl.The goal was MAAS grade ≤3.The SBP,DBP,HR,R,SaO2,MAAS grade,in 4 hrs,8 hrs,24 hrs,and 48 hrs after the medicine were taken.The amount of propofol used,the fee,regain consciousness time and the satisfactory degree of sedation were recorded after taking medicine in 48 hrs.Results There were no significant differences of SBP,DBP,HR,RR,SaO2 between two groups at different time points(F=12.34,15.13,11.46,13.52,13.15,P0.05).After 8 hrs sedation treatment,the SBP was significantly decreased,while the SaO2 was significantly increased(t=8.22,13.11,12.53,9.12,PO.05).After 48 hrs treatment,the amount of propofol used in propofol combined with fetanyl group was less than those used in propofol group,and the regain consciousness time was also decreased.But the satisfactory degree of sedation was higher than that in group propofol(t=21.32,14.52;χ2=7.50,P0.05).Conclusions Propofol combined with fentanyl was a good therapeutic regimen for patient with consciousness by mechanical ventilation.
