胃癌切除术后患者预后的相关因素分析

目的:探讨影响胃癌切除术后患者预后的相关因素。方法:回顾分析江苏大学附属武进医院2007年4月—2013年5月收治的489例行手术治疗的胃癌患者的临床和病理资料,采用单因素和多因素分析各因素与预后的相关性。结果:单因素分析显示,年龄、肿瘤大小、组织学类型、肿瘤浸润深度、手术方式、淋巴结转移个数、术前癌胚抗原(carcinoembryonic antigen,CEA)水平升高、是否存在远处转移与预后显著相关(P0.05)。多因素分析显示,组织学类型、淋巴结转移个数、术前CEA水平升高、是否存在远处转移是影响胃癌切除术后患者预后的独立因素(P0.05)。结论:肿瘤组织学类型、淋巴结转移个数、是否存在远处转移、术前CEA水平升高是影响胃癌切除术后患者预后的重要因素。

Related Factors of Postoperative Prognosis in Patients with Gastric Cancer

Objective：To investigate the related factors of postoperative prognosis in patients with gastric cancer.Methods：The clinical and pathological data of 489 patients with gastric cancer,who underwent surgical treatment from Apr 2007 to May2013in Wujin Hospital,Jiangsu University were retrospectively analyzed,and the prognosis was evaluated with univariate and multivariate analysis.Results：In the univariate analysis,age,tumor size,histological type,depth of invasion,operation method,number of lymph node metastasis,carcinoembryonic antigen（CEA）level increases preoperatively,the existence of distant metastasis were found to be significantly associated with the cumulative survival rate（P〈0.05）.Multivariate Cox regression analysis showed that histological type,number of lymph node metastasis,preoperative increase of CEA level,existence of distant metastasis were the independent factors which affected postoperative outcomes of patients with gastric cancer（P〈0.05）.Conclusions：Histological type,number of lymph node metastasis,existence of distant metastasis and preoperative increase of CEA level are the main factors that affect the postoperative prognosis of patients with gastric cancer.
