呼吸内科应用抗菌药物的干预对照研究

目的通过抗菌药物应用的干预对照研究，探索实施抗菌药物合理使用方案的可行性。方法选取我院呼吸内科2007年11月的133例住院病例为非干预组，2008年11月的75例为干预组，应用我院制定的合理用药干预方案进行干预。比较2组抗菌药物使用率、抗菌药物注射剂使用率、平均抗菌药费、平均住院天数及患者正确用药知晓率。结果抗菌药物使用率、抗菌药物注射剂使用率从干预前的84．21％，70．68％分别下降至干预后的72．00％，52．00％（P〈0．05，P〈0．01）；干预组抗菌药费为（1068．81±97.41）元，显著低于非干预组抗菌药费（1867．43±113.53）元（P〈0．01）；干预组与非干预组住院天数分别为（12．33±1.35）d、（18．57±1．31）d；患者正确用药知晓率从88．72％上升到干预后的98．67％，2组差异有高度显著性（P〈0．01）。结论抗菌药物合理使用方案干预临床用药，对改善合理用药实践是有效的和可行的。

Intervention control study, on the rational application of antimicrobial agents in department of respiratory

Objective To evaluate the feasibility of rational application of antimierobial agents in department of respiratory by intervention control study.Methods 133 patients who were discharged from hospital on November 2007 were as non-interfering group, 75 patients who were discharged from hospital on November 2008 were as interfering group. The use of antibiotics and injectable antibiotics,the cost of antibiotics,the average hospitalization days and patients＇correct indication knowledge in two groups were compared.Results The use of antibiotics and injectable antibiotics dropped from 84.21% ,70.68%before intervention to 72.00% ,52.00% （P〈0.05, P〈0.01 ）, Antibiotic costs dropped from（ 1 867.43 ± 113.53 ）yuan before intervention to（ 1 068.81 ± 97.41 ）yuan.The average hospitalization clays in interfering group and non-interfering group were （ 12.33 ± 1.35 ）and （ 18.57 ± 1.31 ）days respectively, Patients bad correct indication knowledge increased from 88.72%before intervention to 98.67%.Conclusion The rational antimicrobial application in clinical practice is effective and practical.
