不稳定型心绞痛介入治疗临床路径的应用评价

目的:探讨临床路径在不稳定型心绞痛(UAP)介入治疗患者中的应用情况。方法:基于美康合理用药临床药学工作站,对临床路径在我院UAP介入治疗患者中的应用进行比较研究。结果:2010年实施临床路径后,UAP介入治疗患者的住院时间减少了2.05d(P<0.01),人均总治疗费用降低了3363.26元(P<0.01)。在药物方面,指南推荐的主要治疗药应用情况差异无统计学意义;辅助用药中,2010年中药制剂的种类、奥美拉唑的用量显著降低(P<0.05),而磷酸肌酸钠、曲美他嗪的用量显著升高(P<0.01)。结论:实施临床路径可有效缩短UAP介入治疗患者的住院时间,降低总住院费用及药品费用。严格遵循指南、合理选用疗效优越的新型治疗药物等临床路径药物管理是保障临床路径顺利实施的重要环节之一。

Research on the Application of Clinical Pathway in Patients with Unstable Angina Pectoris undergoing Interventional Therapy

OBJECTIVE：To investigate the application of clinical pathway in patients with unstable angina pectoris（UAP）undergoing interventional therapy.METHODS：Comparative study on the application of clinical pathway in UAP patients undergoing interventional therapy was performed on the basis of Mecdex clinical pharmacy working station.RESULTS：After enforcing clinical pathway in 2010,the hospitalization days and total treatment cost of UAP patients undergoing interventional therapy decreased by 2.05 d and 3 363.26 yuan（P0.01）.There was no significant difference in drugs recommended by guideline,while the types of TCM preparation and consumption of omeprazole in 2010 were significantly decreased（P0.05）.The consumption of creatine phosphate sodium and trimetazidine was significantly increased（P0.01）.CONCLUSION：The implementation of clinical pathway can effectively shorten their hospitalization time and control total hospitalization cost and medical cost in UAP patients.The clinical pathway of drug management approach,for example,using drugs strictly abided by guideline,using novel drugs with better effect,may be one of the key points of the successful application of clinical pathway.
