从中焦论治抑郁症

肝气郁结是抑郁症的主要病机,而抑郁症表现的情志抑郁往往并非由肝气郁结所致,多种迟滞症状是由于脾失健运,清阳不升,神机失养导致的。在抑郁症的中医辨证过程中,应当注意将情志抑郁作为辨病要点,而非辨证要点,不能简单地将情志抑郁归结为肝气郁滞而忽略脾虚的核心作用。脾虚肝郁是抑郁症的核心病机,且脾虚是病机关键,贯穿抑郁症发病始终,故抑郁症的治疗当更注重从中焦论治,治肝实脾,且健脾重于疏肝。

Depression of liver qi is the main pathogenesis of depression,while the emotional depression is often not caused by depression of liver qi.Many kinds of retardation symptoms are caused by dysfunction of spleen in transportation,lucid yang failing to rise and the lack of nourshing of spiritual mechanism.In the process of TCM syndrome differentiation of depression,we should pay attention to emotional depression as the key point of disease differentiation rather than syndrome differentiation.We can't simply attribute emotional depression to stagnation of liver qi and ignore the core role of spleen deficiency.Spleen deficiency and liver depression are the core pathogenesis of depression in Chinese medicine,and spleen deficiency is the key pathogenesis,which is throughout the onset of depression,so the treatment of depression should pay more attention to the treatment from the middle jiao,and treat liver and strengthen spleen.Strengthening spleen is more important than soothing liver.
