温阳健脾法治疗糖尿病周围神经病变

目的:基于脾气散精探讨温阳健脾法治疗糖尿病周围神经病变的临床疗效。方法:86例糖尿病神经病变患者随机平均分为对照组和研究组。对照组给予来氟米特,研究组在对照组治疗基础上给予温阳健脾法治疗。比较两组患者治疗前后血清炎性因子水平[白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)],氧化应激指标[超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)],纤维化指标[Ⅳ型胶原(collagen typeⅣ,CⅣ)、结缔组织生长因子(connective tissue growth factor,CTGF)],肾功能[血肌酐(serum creatinine,SCr)、β2-微球蛋白(β2-microglobulin,β2-MG)、尿素氮(urea nitrogen,BUN)],腓总神经和胫神经传导速度。结果:治疗后,研究组血清IL-6、TNF-α水平低于对照组(P<0.05);研究组SOD水平高于对照组(P<0.05),MDA水平低于对照组(P<0.05);研究组CⅣ、CTGF水平低于对照组(P<0.05);研究组β2-MG、BUN、SCr水平低于对照组(P<0.05);研究组腓总神经和胫神经传导速度高于对照组(P<0.05)。结论:温阳健脾法治疗糖尿病周围神经病变疗效显著,可降低炎性因子水平,改善肾功能和纤维化指标,提高神经传导速度。

Treatment of Diabetic Peripheral Neuropathy with TCM Wenyang Jianpi Method

Objective: To explore the clinical efficacy of TCM Wenyang Jianpi method in treating diabetic peripheral neuropathy based on spleen-qi-dispersing. Methods: 86 patients with diabetic neuropathy were randomly divided into control group and study group. The control group was given leflunomide,and the research group was given the warm Yang Jianpi method on the basis of the treatment of the control group. Comparison of serum inflammatory factor levels [interleukin-6( IL-6),tumor necrosis factor-α( TNF-α) ],oxidative stress indicators before and after treatment in the two groups of patients[Superoxide dismutase( SOD),malondialdehyde( MDA) ],fibrosis indicators [collagen type Ⅳ( CIV),connective tissue growth factor( CTGF) ],Renal function[serum creatinine( SCr),β2-microglobulin( β2-MG),urea nitrogen( BUN) ],and the conduction velocity of the common peroneal and tibial nerves. Results: After treatment,serum levels of IL-6 and TNF-α in the study group were lower than those in the control group( P < 0. 05);SOD levels in the study group were higher than those in the control group( P < 0. 05),and MDA levels were lower than those in the control group( P < 0. 05);CⅣ and CTGF levels in the study group were lower than those in the control group( P < 0. 05);β2-MG,BUN,and SCr levels in the study group were lower than those in the control group( P <0. 05);Control group( P < 0. 05). Conclusion: TCM Wenyang Jianpi method is effective in treating diabetic peripheral neuropathy,which can reduce the levels of inflammatory factors,improve renal function and fibrosis indexes,and increase nerve conduction speed.
