我院临床常见病原菌耐药性分析

摘    要:
目的:了解我院2008—2009年院内常见致病菌对不同类型抗菌药的耐药情况,为临床合理使用抗菌药提供依据。方法:对临床送检标本中培养分离的常见致病菌:肺炎克雷伯菌、铜绿假单胞菌、大肠埃希菌、白念珠菌、粘质沙雷菌、嗜麦芽窄食单胞菌、金黄色葡萄球菌进行分析,并与2006—2007年度全国细菌耐药监测结果进行对比。结果:我院铜绿假单胞菌(12.32%)对抗菌药的耐药率于全国平均水平类似,金葡菌(5.44%)耐药率略低于全国平均水平,肺炎克雷伯菌(14.24%)、铜绿假单胞菌(12.32%)、大肠埃希菌(11.36%)耐药率高于全国平均水平,MRSA检出率为29.41%,未检出耐万古霉素及替考拉宁菌株;大肠埃希菌、克雷伯菌中产ESBLs检出率分别为6.6%和10.12%。结论:我院细菌耐药情况不容乐观,继续开展细菌耐药监测,对指导临床合理应用以及经验应用抗菌药具有重要意义。

Abstract:
Objective : To analyze the drug resistance against different types of common pathogenic bacteria in our hospital in 2008 -2009 to provide the basis for the rational use of antimicrobial agents. Method:The clinically cultured and isolated specimens for inspection of common pathogens : Klebsiella pneumoniae, Pseudomonas aeruginosa, Escherichia coli, Serratia marcescens, Stenotrophomonas maltophilia, Staphylococcus aureus were analyzed and compared with the 2006 -2007 annual national antimicrobial resistance monitoring results. Result: The resistance rate Pseudomonas aeruginosa (12.32%) was similar to the national average, Staphylococcus aureus (5.44%) was slightly lower than the national average,the resistance rates of Klebsiella pneumoniae ( 14.24% ) , Pseudomonas aeruginosa ( 12.32% ) and Escherichia coli (11.36%) were higher than the national average. MRSA detection rate was 29.41% but no drug resistance was detected against vancomycin and teicoplanin strains. The detection rates of Escherichia coli, Klebsiella producing ESBLs were 6.6% and 10.12%. Conclusion:The overall drug resistance against bacteria was not considered to be well handled in our hospital so that it should continue to monitor the antimicrobial resistance, which meant a lot in guiding clinical antimicrobial drugs use.
