乙型肝炎免疫球蛋白阻断乙肝病毒母婴垂直传播临床研究

目的:探讨孕晚期乙型肝炎免疫球蛋白(HB IG)对阻断乙肝病毒(HBV)母婴垂直传播的临床效果。方法:对杭州市拱墅区近郊农村乙肝病毒携带家庭高发人群进行乙肝病毒母婴垂直传播情况调研。阻断组:18例HBsAg(+)孕晚期(7、8、9月)乙型肝炎免疫球蛋白(HB IG)400 IU/月肌注。非阻断组:20例HBsAg(+)孕妇自然孕产。生后两组婴儿均按正规乙肝疫苗接种。1岁后做肝功、乙肝抗体检测、对比调研。结果:阻断组:阻断乙肝病毒垂直传播成功率83.9%,失败率16.1%。非阻断组:阻断乙肝病毒垂直传播成功率42.9%,失败率51.8%。阻断组与非阻断组间比较有统计学意义(P<0.01)。结论:乙肝病毒携带者孕晚期乙型肝炎免疫球蛋白阻断乙肝病毒母婴垂直传播有显著疗效。

Clinical study on HBIg interrupting HBV of mother- infant perpendicular dissemination

Objective:To explore the clinical study on HBIg interrupting HBV of mother-infant perpendicular dissemination.Methods:HBV carriers in countryside family were investigated on mother and infant erpendicular dissemination,including interruption group(18 cases of positive HBV women given HBIg 400 IU per month i.m.in later gestation period) and non-interrupting group(20 cases without HBIg immunity).Neonates in two groups were given routine HB inoculation to determine and compare liver function and HBAb in neonates aged 1 year.Results:The interruption rates was 83.9% and 42.9%,unsuccessful rate was 16.1% and 51.8% in interruption group and non-interrupting group,repectively(P<0.01).Conclusion:HBIg immunity can effectively decrease the rate of intrauterine transmission and maternal-infant transmission of HBV in later gestation period.
