宫颈腺癌86例临床分析

目的探讨宫颈腺癌的临床特点和预后影响因素。方法回顾性分析2000年1月至2009年1月解放军总医院妇产科收治的86例宫颈腺癌患者的临床资料。结果 86例患者5年总生存率为62.3%,其中临床期别Ⅰ、Ⅱ、Ⅲ、Ⅳ期患者的5年生存率分别为83.3%、62.5%、13.7%和0。淋巴结转移患者5年生存率为21.4%,无淋巴结转移患者5年生存率85.1%,两者比较,差异有统计学意义(P0.05);血CA125、CA199水平升高均合并淋巴结转移;保留卵巢患者总5年生存率为80.0%,不保留卵巢患者总5年生存率是59.7%,两者比较,差异无统计学意义(P0.05)。结论临床期别、淋巴结转移是影响宫颈腺癌预后的主要因素;早期宫颈腺癌保留卵巢不影响生存率;CA125、CA199的检测对宫颈腺癌预后判断有一定的参考价值。

Clinical analysis of 86 cases of cervical adenocarcinoma

Objective To analysis the clinical characteristics,treatment and prognosis of cervical adenocarcinoma.Methods Data of 86 patients with adeno-cervical cancer who were admitted to Department of Obstetrics and Gynecology,General Hospital of PLA from Jan 2000 to Jan 2009 were analyzed retrospectively.Results The overall 5-year survival rate was 62.3%,It was 83.3%,62.5%,13.7% and 0 for stages Ⅱ,Ⅲ and Ⅳ,respectively.5-year survival rate of pelvic lymph node metastases was 21.4%,while it was 85.1% for those who had no lymph node involved（P0.05）.Advanced stages and increased levels of CA125and CA199 would be prone to have pelvic lymph node metastases.Conclusions This retrospective study suggests that FIGO stages and pelvic lymph node metastasis are independent factors of the survival rate for cervical adenocarcinoma patients.Combination CA125with CA199 can improve the sensitivity of diagnosis.
