阴道内放置PGE_2用于足月妊娠引产351例疗效观察

目的　比较 PGE2 阴道栓剂及催产素静滴引产的效果。方法　将 5 85例同样条件孕妇 ,随机分两组 ,分别给PGE2 阴道栓剂和催产素静脉滴注引产。结果　前列腺素组及催产素组引产成功率分别为 96 .3%和 79.92 % ;临产发动时间及总产程分别为 (2 .70± 0 .6 3) h,(3.0 1± 3.41) h和 (3.91± 0 .71) h,(6 .0 3± 3.5 0 ) h;剖宫产率分别为6 .8%和 18.18% (P<0 .0 1) ,有显著差异。结论　 PGE2 栓剂用于足月妊娠引产效果显著、安全、方便 ,对促宫颈成熟意义重要 ,可缩短住院天数 ,减少住院费用。

Observation on Effectiveness of Prostaglandin E2 Vaginal Insert For Induction of Labour

Objective\ To compare the effectiveness of labour induction by using either prostaglandin E2 vaginal gel, or intravenous drip of oxytocin. Methods\ 585 hospitalized patients at ≥37 weeks requiring labour induction and having the same condition were randomly divided into two groups and given prostaglandin E2 vaginal gel (the study group ) and oxytocin intravenous drip(the control group) respectively to induce labour. Results\ Success rate of prostaglandin E2 in induction of labour was 96.3%,signifcantly higher than that of oxytocin(79.92%)(P<0.01).The interval between administration of prostaglandin E2 and onset of labour and the duration of labour were 2.7±0.63 hours and 3.01±3.41hours, respectively, wherease those of the control group were 3.91±0.71 hours and 6.03±3.50 hours respectively(P<0.01), with significant difference found between the two groups. Cesarean section rate in the study group was 6.8%, signicantly lower than that in the control group(18.18%,P<0.01). Mean birth weight and incidence of neonatal asphyxia in the two groups were similar (P>0.05) .Conclusion\ Administration of prostaglandin E2 vaginal gel in induction of term pregnancy appeared to be safe, effective and convenient and , especially, the effect of prostaglandin E2 for promoting cervical ripening was most significant. The use of prostaglandin E2 gel would result in shortened hospitalization and less expense.
