鼻咽癌原发灶热疗对颈淋巴结转移灶的影响

摘    要:[目的]观察鼻咽癌原发灶局部热疗后对颈淋巴结转移灶的影响.[方法]98例初治鼻咽癌患者按年龄、性别、T、N、M共5个因素配对分为热放组与单放组.两组均常规放疗,热放组患者每周做1~2次鼻咽腔内微波热疗,两组中各28例治疗前后做了细胞免疫功能检查.[结果]热放组原发灶肿瘤完全消退率提高的同时,颈淋巴结转移灶的消退率也较对照组高,分别为78.95%与55.26%,P<0.05.热放组治疗后各项细胞免疫指标均较治疗前升高,而单放组NK活性与淋巴细胞转换率却呈下降趋势,但差异均无统计学意义(P>0.05).[结论]对鼻咽癌原发灶进行腔内微波热疗能提高放疗时颈淋巴结转移灶的消退率.

Abstract:To observe the effect of hyperthermia for primary lesion in nasopharyngeal carcinoma on neck lymphatic metastases.Ninety-eight cases of untreated nasopharyngeal carcinoma were divided into thermoradiotherapy group and radiotherapy alone group according pair matching of age,sex,and T,N,M staging.Conventional radiotherapy was carried out in two groups.Patients in thermoradiotherapy group were heated with microwave in nasopharynx once or twice a week.Twenty-eight patients of two groups in each were made cell immunity inspection before and after treatment.CR rate of neck lymphatic metastases in thermoradiotherapy group(78.95%) were higher than that in the radiotherapy alone group(55.26%)(P<0.05).In thermoradiotherapy group cell immunity improved after treatment.While, NK activity and lymphocyte transformation rate went down in radiotherapy alone group.But no significant difference were found (P>0.05).[Conclusion]Microwave hyperthermia for primary lesion in nasopharyngeal carcinoma could raise the response rate of neck lymphatic metastases.
