无抽搐电休克对难治性精神分裂症认知功能影响的对照研究

摘    要:目的观察无抽搐电休克(MECT)治疗对难治性精神分裂症患者认知功能的影响。方法将60例难治性精神分裂症患者随机分为研究组和对照组,研究组进行抗精神病药物联合MECT治疗,对照组进行单纯药物治疗。在治疗前后分别进行威斯康星卡片分类(WCST)、连线试验A和B、数字广度(DST)、数字符号和临床记忆量表(CMS)检测,评价其认知功能。结果研究组患者治疗后WCST测试的应答时间、总应答数及错误应答数较治疗前明显减少(P<0.05)。连线试验A、B中,研究组患者的完成时间较治疗前显著缩短(P<0.05)。数字符号、数字广度及CMS两组治疗前后比较差异均无显著性。结论 MECT治疗后,难治性精神分裂症患者的认知功能有一定程度的改善,尤其是患者的记忆功能及注意力功能。

Abstract:Objective To explore the effects of modified electroconvulsive therapy(MECT) on cognitive functions in refractory schizophrenia.Methods A total of 60 cases of refractory schizophrenia were randomly allocated to MECT group(a study group) and antipsychotic drug group(a control group).The Wisconsin Card Sorting Test(WCST),trail making test A and B,Digit Span Test(DST),Digit-Symbol Substitution Test and Clinical Memory Score(CMS) were performed before and after treatment for assessing the cognitive functions of the patients in the two groups.Results Compared to pre-treatment in the study group,the total reponsive time,number of total trials and total reponsive errors of WCST reduced after treatment;there was statistical difference between them(P<0.05).In trail making test A and B,the finished time was significantly shorter,compared post-treatment with pre-treatment(P<0.05) in the study group.In the two groups,there were no significant differences in DST,Digit-Symbol Substitution Test and CMS,compared pre-treatment to post-treatment.Conclusion The cognitive functions of refractory schizophrenia have some improvements after MECT,especially in memory and attention.
