急诊内镜介入与开腹手术治疗结石性急性梗阻化脓性胆管炎的对比观察

目的比较急诊内镜介入与开腹手术治疗结石性急性梗阻化脓性胆管炎（AOSC）疗效。方法总结分析1997年1月以来经急诊内镜介入治疗32例与同期急诊开腹手术的43例结石性AOSC的治疗效果和病情缓解时间。结果急诊内镜介入治疗和急诊开腹手术治疗AOSC总治愈率、死亡率、转诊率和合并症发生率分别为81．3％（26／32）和62．8％（27／43）、6．3％（2／32）和16．3％（7／43）、12．5％（4／32）和20．9％（9／43）、9．4％（3／32）和6．3％（7／43）。急诊内镜介入治疗在腹痛的缓解时间、胃肠功能的恢复时间、引流管的解除时间、总住院时间等方面明显短于开腹手术治疗，两组比较差异有显著性；急诊内镜介入治疗在休克纠正时间、体温下降时间、黄疸减轻缓解时间与开腹手术治疗进行比较，差异无显著性。结论急诊内镜介入结石性AOSC在总体疗效和安全性方面优于开腹手术治疗。
