2002年－2011年平安县围产儿死亡情况分析

目的：通过对2002年－2011年平安县妇幼保健院围产儿死亡情况的分析，总结平安县在围产期保健方面的特点及变化规律，以进一步提高国产儿保健水平，降低围产儿死亡率。方法：分析2002年1月－2011年12月平安县妇幼保健院160例死亡围产儿的病案资料。结果：2002年1月—2006年12月国产儿死亡率21．85％o；2007年1月—2011年12月围产儿死亡率8．00‰；平均死亡率14．69‰。呈逐年下降趋势。2002年1月—2006年12月新生儿死亡60例，其中家中死亡15例，占25．00％；转送途中死亡10例，占16．67％，各级医院死亡35例，占58．33％。2007年1月-2011年12月新生儿死亡21例，其中家中死亡3例，占14．29％；转送途中死亡2例，占9．52％；各级医院死亡16例，占76．19％。新生儿死亡中出生缺陷为最主要的死亡原因。死胎死产中前三位死因分别为：重度子痫前期／子痫、出生缺陷、内科合并症。结论：加强婚前保健和孕前保健，做好出生缺陷三级预防，加强对农村地区及城市流动人口孕产妇的健康教育及规范的产前检查是降低围产儿死亡率的关键。

Investigation of Perinatal Mortality in Pingg.n County

Objective： To investigate the perinatal mortality during 10 years （2002--2011） in Pingkn county to improve the level of health care and reduce perinatal mortality. Methods：Perinatal death data of 160 cases from Jan 2002 to Dec 2011 in our center were reviewed retrospectively, and the reasons and place of death were recorded. The results was analyzed by trend test, a =0.05. Results：21.85 %0 of perinatal mortality rate was during Jan 2002--Dec 2006, in which 25.00% （n = 15）, 16.67% （n = 10） and 58.33% （n =35） died in home, on way and at level hospital respectively. During Jan 2007--Dec 2011, 21 cases were dead, of them 14.29% （ n = 3 ）, 9.52% （n =2） and 76.19% （n = 16） died at home, on way and at level hospital respectively. The death reason of newbern is importantly the birth defects, including severe preeclampsia/eclampsia and medical complications. Conclusions：Premarital health and preconception care would be strengthened; carrying out effective intervention to promote the self - conscious of citizens for three - step - prevention mode of birth defect in rural and urban is necessary.
