改良术式与传统术式治疗腮腺浅叶良性肿瘤的临床对比研究

目的探讨改良术式与传统术式治疗腮腺浅叶良性肿瘤的临床效果,以期提高临床治疗水平。方法将苏州大学附属第一医院于2010年1月至2013年2月收治的90例腮腺浅叶良性肿瘤患者依据随机数字表法分为两组:采用传统术式治疗的45例为传统术式组,采用改良术式治疗的45例为改良术式组,用SPSS 13.0统计软件进行统计分析两组治疗后的临床效果。结果改良术式组手术时间、术中出血量、切口长度、解剖面神经分支数均少于传统术式组,差异有统计学意义(P<0.05);改良术式组面瘫、Frey综合征、涎瘘或积涎、耳周皮肤麻木、面部凹陷畸形的发生率均低于传统术式组(P<0.05);两组患者治疗后2周、3个月、12个月在面瘫、面部畸形各分级发生率的比较,差异均有统计学意义(P<0.05)。结论改良术式治疗腮腺浅叶良性肿瘤的临床效果满意,能有效缩短手术时间,减少术中出血,缩短手术时间,并且能降低并发症。

Clinical Comparative Study of Improved Operation and Conventional Surgical Treatment of Benign Parotid Superficial Lobe Tumor

Objective To investigate the clinical effect of improved operation and conventional surgical treatment of benign parotid superficial lobe tumor, in order to improve the level of clinical treatment. Methods From the First Affiliated Hospital of Suochow University in Jan. 2010 to Feb. 2013,90 cases of benign parotid superficial lobe tumor were included in the study,which were divided into two groups according to the random-number table method, the control group of 45 patients, receiving the traditional surgical treatment, the observation group of 45 patients, receiving the modified surgical treatment. The clinical effect of the two groups was analyzed by SPSS 13.0 statistical software. Results Operative time, blood loss, incision length, the facial nerve branches number of the observation group were less than the control group, the difference was statistically significant（ P 〈 0.05 ）. The incidence of facial paralysis, Frey syndrome, salivary fistula or plot saliva, ear adjacent skin numbness and facial deformity of observation group were lower than the control group, the difference was statistically significant （ P 〈 O. 05 ）. After 2 weeks, 3 months, 12 months of treat- ment, differences in facial paralysis, facial deformity incidence of different grades between the two groups were statistically significant （ P 〈 O. 05 ）. Conclusion The modified surgical treatment of benign parotid superficial lobe tumor has satisfactory clinical results, which can effectively shorten the operation time, reduce blood loss, and reduce the complications.
