MSCT灌注成像对乳腺肿块良恶性的评价

摘    要:目的:通过应用MSCT灌注成像获得的乳腺良恶性肿块灌注参数的分析评价乳腺肿块的良恶性。方法:18例体检或钼靶检查发现乳腺肿块性病变的女性患者,其中良性病变5例,乳腺癌13例,采用GElightspeed16层CT灌注成像分别测量血流量(BF),血容量(BV),平均通过时间(MTT)和表面通透性(PS),采用t检验统计学分析肿块灌注参数。结果:乳腺癌组BF、BV和PS均值分别高于对侧正常乳腺组BF、BV和PS均值,两组间BF、BV和PS均值差异有统计学意义(P<0.05),而两组间MTT均值差异无统计学意义(P>0.05)。良性病变BF、BV和PS均值分别低于乳腺癌组BF、BV和PS均值,与乳腺癌组之间BF、BV和PS均值差异有统计学意义(P<0.05),而两组间MTT均值差异无统计学意义(P>0.05)。结论:CT灌注成像能获得反映肿瘤组织血流动力学变化的定量参数,反映乳腺病变的血液灌注特点,有助于乳腺肿块良恶性的评价。

Abstract:Objective:To evaluate the benign and malign breast tumors by analysis of perfusion parameters in tumors with perfusion MSCT imaging.Methods:18 patients with breast tumors were found by the examination or mammography,5 patients with benign diseases,and 13 patients with carcinomas.All patients underwent GE light speed 16 slices perfusion CT for the measurement of tumor blood flow,blood volume,mean transit time,and permeability surface area product.Student t testing was used to investigate relationships between perfusion parameters in breast tumors.Results:The mean values of BF,BV,MTT and PS in carcinoma were higher than those of BF,BV and PS in opposite side normal breast of carcinoma respectively.The differences of mean values of BF,BV and PS between carcinoma and normal breast were statistically significant(P<0.05),and the difference of mean value of MTT between the two groups was not statistically significant(P>0.05).The mean values of BF,BV and PS in benign tumors were lower than those of BF,BV and PS in carcinoma respectively.The differences of mean values of BF,BV and PS between carcinomas and benign tumors were statistically significant(P< 0.05),and the difference of the mean value of MTT between the two groups was not statistically significant(P>0.05).Conclusion:Perfusion CT can obtain quantitative perfusion parameter that may response the hemodynamic change of breast tumors,and it may response the perfusion characteristic of breast tumors.Perfusion CT may be used to evaluate whether the innocence or malignancy of breast tumor.
