放疗加顺铂单药同步化疗治疗中晚期鼻咽癌临床疗效观察

目的：目的评价放疗加顺铂单药同步化疗治疗中晚期鼻咽癌临床疗效和不良反应。方法将中晚期鼻咽癌123例按抽签法随机分为研究组63例和对照组60例。研究组给予放疗合并第1、4、7周用单药顺铂（DDP）80mg/m2，分次d 1～3天静滴，对照组行单纯放疗。两组放疗技术和放疗剂量均相同。结果研究组与对照组比较：近期疗效、远处转移率、皮肤放疗反应、口咽部粘膜放疗反应及血液毒性反应差异均无统计学意义（ P ＞0．05）；局部复发率（11．1％）低于对照组（25％）（ P ＝0．045）；1、3年积累生存率（96．7％、78．6％）高于对照组（92．7％、66．7％）（ P ＝0．042）。结论放疗加顺铂单药同步化疗治疗中晚期鼻咽癌可以提高局部控制率和生存率，值得临床进一步研究。

CLINICAL EFFECT OF RADIOTHERAPY AND CISPLATIN IN TREATING NASOPHARYNGEAL CARCINOMA AT INTERMEDIATE AND ADVANCED STAGES

Objective To evaluate clinical effects and side effects of synchronized treatment with chemo-therapy and cisplatin on nasopharyngeal carcinoma at intermediate and advanced stages .Methods 123 pa-tients at intermediate and advanced stages of nasopharyngeal carcinoma were randomly divided into the re-search group of 63 cases treated with radiotherapy and additional intravenous drips of 80mg/m2 of cisplatin in three days separately in the 1st ,4th and 7th weeks and the contrast group of 60 cases treated with radio-therapy .The same radiotherapy skills and exposure dose were applied in both groups .Results The curative effect ,the rate of distant metastasis ,the skin reaction of radiotherapy ,the mucous membrane reaction of radiotherapy in pharynx oralis and the reaction of hematotoxicity showed no significant difference between the two groups（ P 〉0 .05） .The local recurrence rate （11 .1% ）in the research group was lower than that （25% ） in the contrast group（ P=0 .045） .The survival rates in the 1st and 3rd years were 96 .7% and 78 . 6% in the research group and 92 .7% and 66 .7% in the contrast group（ P =0 .042） .Conclusion Synchro-nized treatment with chemotherapy and cisplatin can produce good local control rate and survival rate in treating nasopharyngeal carcinoma at intermediate and advanced stages .
