微创侧脑室引流联合脑脊液置换治疗重型脑室出血的临床研究

目的观察微创侧脑室穿刺引流联合腰穿脑脊液置换治疗重型脑室出血的疗效。方法54例重型脑室出血患者随机分组,28例单侧或双侧侧脑室引流治疗者作为对照组,26例单侧或双侧侧脑室引流联合腰穿脑脊液置换治疗者作为治疗组,比较两组治疗前后神经功能缺损评分、治疗效果和治疗后3个月随访Barthel指数。结果两组疗效相近,而治疗组在脑室出血清除时间、生活能力评分方面优于对照组。结论微创侧脑室穿刺引流联合腰穿脑脊液置换是治疗重型脑室出血的有效方法。

Clinical research on severe intraventricular hemorrhage treated with lateral ventricle drainage and replacement of cerebrospinal fluid

Objective To observe the clinical effect of lateral ventricle drainage and replacement of cerehrospinal fluid on severe intraventrieular hemorrhage. Methods 28 patients were treated with unilateral or bilateral lateral ventricle drainage. 26 patients were treated with lateral ventricle drainage and replacement of cerebrospinal fluid. The neurological deficit scores, efficacy and Barthel index were observed. Results There were no statistical difference of effectiveness and mortality rate between the two groups （P〉0.05）, Removal of ventricular hemorrhage and the ability of life score of the treatment group were significantly different from that of control group （P〈0.05）. Conclusion It is effective to treat severe intraventricular hemorrhage with minimally invasive lateral ventricle drainage and replacement of cerebrospinal fluid.
