22例重症甲型H1N1流感的临床特点

目的分析重症甲型H1N1流感患者临床特点。方法收集2009年11月1日～2010年2月6日住院的重症甲型H1N1流感患者22例的临床资料,从患者性别、年龄分布、是否接种疫苗、临床特征、主要并发症及治疗效果等方面进行分析。结果①所有患者均未接种疫苗;②重症患者中青壮年比例最高,特别是体重偏胖的青壮年;③发病症状重,并发症多;主要并发症有脑炎、肺炎,而以重症肺炎居多,危重患者多有肝功、肾功能、心肌酶异常,死亡患者死因主要为重症肺炎,呼吸衰竭;④治疗效果：单纯重症患者治疗效果较好,危重症患者病死率较高。结论体重偏重的青壮年、孕妇、有基础病变者是重症、危重症甲型H1N1流感的高危人群,危重者病死率高,流行季节高危人群应及时接种疫苗。

The clinical characteristies of 22 confirmed cases with severe Pandemic H1N1 influenza

Objective To investigate and summarise the characteristics of clinical manifestations and treatments of severe pandemic H1N1 influenza.Methods The dynamic observations were carried out on the characteristics of clinical manifestations of 22 confirmed severe cases who came to hospitals for pandemic H1N1 influenza between November 1,2009 to February 6,2010.Results ①All of the 22 cases were not vaccinated.②The disease incidence was highest in the young adults,especially in the obese.③Severe cases usually have many complications such as encephalitis and pneumonia,most of which were severe pneumonia.Most of the severe cases have abnormalities in hepatic function,renal function and creatase.And the complications of death cases were mainly severe pneumonia,which leads to respiratory failure.④The therapeutic efficacies from simple severe cases were better,to be sorry,the fatal rate of the danger severe cases was rather higher.Conclusion The young adults with higher body weight,pregnant women and the patients who had foundation diseases were high-risk groups of severe cases.And the danger severe cases had extremely higher fatality.The high-risk group should be vaccinated in time in the epidemic seasons.
