摘    要:目的:观察血必净对烧伤合并急性肾损伤的临床作用,并探析相关作用机制。方法:对本院2012年9月至2015年9月85例不同程度烧伤患者资料进行回顾性分析,根据不同是否使用血必净将85例患者分为治疗组(使用血必净)39例,余下46例设为常规组(未使用血必净),同时将25例健康体检者设为对照组,分别采集3组尿液和血液,检测尿KIM-1及NAG水平及血尿素氮(BUN)、肌酐(Cr)水平。结果:1)与健康对照组相比,烧伤合并肾损伤患者BUN及Cr明显升高,具有统计学意义(P<0.05)。治疗组和常规组,治疗前BUN及Cr比较无统计学意义(P>0.05),经过治疗后治疗组BUN及Cr下降的趋势较常规组明显,差异具有统计学意义(P<0.05)。2)治疗前相比,治疗后治疗组和常规治疗组患者尿KIM-1及NAG水平均有明显下降(P<0.01),其中治疗组上述指标均显著低于常规治疗组(P<0.05)。3)尿KIM-1与BUN、Cr均呈正相关关系;尿NAG与BUN、Cr亦呈正相关关系,尿KIM-1与尿NAG也具有正相关性,结论:血必净对烧伤患者合并急性肾损伤具有理想疗效,其机制可能与降低尿KIM-1和NAG的表达有关。

Abstract:To observe the curative effect and mechanism of Xuebijing injection in burn injury complicated with acute kidney injury.Methods:A total of 85 patients(enrolled during September 2012 to September 2015) with different degrees of burning were retrospectively analyzed, and were divided into observation group(used Xuebijing injection, 39 cases) and conventional group (unused Xuebijing injection, 49 cases). Also, 25 healthy people were set as control group. To collect urine and blood, detect urine KIM-1, NAG levels and blood urea nitrogen (BUN), and creatinine (Cr) levels in the three groups.Results:1) compared with healthy group, BUN and Cr increased significantly in patients with burning injury complicated with renal injury, (P<0.05). Before treatment, BUN and Cr were of no statistical difference in the observation group and conventional group, (P>0.05). After treatment, the downward trend of BUN and Cr in the observation group were more obvious than than of the healthy group, and the differences were with statistically significance (P<0.05). 2) after treatment, urinary KIM-1 and NAG levels were significantly decreased in the observation group and conventional group(P<0.01), and the indexes of observation group were significantly lower than that of the conventional treatment group (P<0.05); 3) urine KIM-1 were both positive correlated with BUN and Cr, and also, urine NAG had positive correlation with BUN and Cr, and urine NAG also had positive correlation with urine KIM-1.Conclusion:Xuebijing injection has an ideal curative effect for patients with burns and acute kidney injury. This might be related to its inhibition on the expression of urine NAG and KIM-1.
