磺达肝癸钠和低分子肝素钙对不稳定型心绞痛的疗效与安全性比较

目的观察比较磺达肝癸钠和低分子肝素钙对不稳定型心绞痛的临床疗效及安全性。方法 130例不稳定型心绞痛患者随机分为对照组(n=62)和治疗组(n=68),两组基础治疗相同,对照组给予低分子肝素钙0.4mL皮下注射,每12 h一次,共7 d;治疗组给予磺达肝癸钠2.5 mg·d^(-1)皮下注射,共7 d。观察治疗7 d时及30 d内心脏事件发生情况,比较两组疗效及安全性。结果治疗7 d后,治疗组与对照组心血管事件(再发心绞痛、Q波性心肌梗死、死亡、室速、室颤)及严重出血的发生率比较,无显著差异(P>0.05);轻度出血并发症例数均较对照组明显减少(P>0.05)。治疗30 d后,治疗组的心血管事件发生率与对照组相比无显著差异(P>0.05),严重出血及轻度出血并发症例数均较对照组明显减少(P<0.05)。结论磺达肝癸钠治疗不稳定型心绞痛安全有效,且出血发生率较低分子肝素钙明显降低。

Comparison of efficacy and safety of fondaparinux and nadroparin in treatment of unstable angina

AIM To evaluate the efficacy and safety of fondaparinux and low-molecular-weight heparin in the treatment of unstable angina.METHODS A total of 130 patients with unstable angina were randomly divided into treatment group(n = 68) and control group(n = 62).Besides the basic treatment,nadroparin0.4 mg was administrated to the control group subcutaneously every 12 hours for 7 days.The treatment group was given fondaparinux 2.5 mg per day subcutaneously for 7 days.Occurrences of cardiac disease were observed for 7and 30 days.RESULTS After 7 days treatment,there was no significant difference in incidence of cardiac events(relapse angina,ST elevate myocardial infraction,death,ventricular tachycardia,ventricular fibrillation),and the incidence of major bleeding was no significant difference in the control group and the treatment group(P > 0.05).The incidence of minor bleeding in the treatment group was obvious depressed(P < 0.05).After 30 days treatment,incidence of cardiac events was no significant difference between two groups(P >0.05).The incidence of major bleeding and minor bleeding in the treatment group were obvious depressed(P <0.05).CONCLUSION Fondaparinux is safe and effective in the treatment of unstable angina,and has lower incidence of hyporrea than the low-molecular-weight heparin.
