辅助生育并发异位妊娠的腹腔镜处理

目的:探讨腹腔镜在诊断、治疗辅助生育并发异位妊娠的应用价值。方法:回顾性分析1999年7月~2004年12月我院妇科微创治疗中心应用腹腔镜诊断、治疗辅助生育并发异位妊娠26例。包括双侧输卵管同时妊娠,宫内、宫外同时妊娠,宫内、宫角同时妊娠等特殊类型异位妊娠。手术包括输卵管切开吸胚术、患侧或双侧输卵管切除术、输卵管间质部切开吸胚术、输卵管造口术、卵巢囊肿剔除术、卵巢修补术、卵泡穿刺放液术、卵巢部分切除术、卵巢电凝止血术、附件切除术、腹腔镜辅助清宫术、盆腔粘连松解术等。结果:腹腔镜下手术成功24例(92.31%),中转开腹2例(7.69%),无手术并发症,无死亡病例。结论:腹腔镜手术能安全有效地应用于辅助生育并发异位妊娠的诊断及治疗,助孕术后适时以腹腔镜作为有效的诊治手段,不仅可明确诊断,早期干预,还可为保守性手术赢得时间,有利于保留生育功能。

Clinical application of laparoscopic surgery in treatment of ectopic pregnancy complicated with assisted reproductive techniques

Objective:To explore the clinical value of laparoscopic surgery in treatment of Ectopic Pregnancy complicated with Assisted Reproductive Techniques(ART).Methods:Retrospective analysis 26 cases of laparoscopic surgery in treatment of ectopic pregnancy complicated with ART from Jul 1999 to Dec 2004.Operations include salpingectomy,salpingotomy,salpingostomy,excision and enucleation of ovarian masses,repair of ovary,perforation of ovary,oophorectomy,follicular puncture,adnexectomy,lysis of pelvic adhesions.Results:24 cases succeeded with laparoscopy,and 2 cases conversed to laparotomy.There was no death and Complications.Conclusion:Laparoscopic surgery can be safely and effectively utilized in treatment of Ectopic Pregnancy complicated with ART.
