延边朝鲜族和汉族农村居民不同类型肥胖指标与高血压的关系

摘    要:目的了解延边地区朝鲜族和汉族农村居民的肥胖现状及不同类型肥胖指标与高血压的关系。方法于2006年8月至2008年8月在延边农村地区采用简单随机抽样的方法先抽取3个镇,然后在各镇内分别抽取2个村作为研究社区,最后在研究社区内采用整群抽样的方法抽取20~70岁朝鲜族和汉族常住居民5838人,其中朝鲜族2852人(占48.9%),汉族2986人(占51.1%),其平均年龄分别为(53.4±10.8)岁和(47.5±11.4)岁。测量身高、体重、腰围(WC)、臀围、血压等指标,分析不同类型肥胖指标对血压水平的影响。结果不同类型肥胖组的高血压、1级高血压、2级高血压、3级高血压的患病率均高于正常体重组(P0.05)。体质指数和不同类型腹型肥胖指标均异常组高血压患病危险性高于任何一种肥胖指标异常组,差异均有统计学意义(P0.01)。Logistic回归分析表明,调整年龄、性别、学历、吸烟和饮酒等因素后,朝鲜族人群的超重、周身型肥胖和以腰围身高比值(WHtR)作为判断标准的腹型肥胖与高血压患病呈正相关,其OR(95%CI)分别为1.736(1.400~2.152)、3.320(2.373~4.647)、1.425(1.161~1.749);汉族人群的超重、周身型肥胖和分别以WC和腰围臀围比值(WHR)作为判断标准的腹型肥胖与高血压患病呈正相关,其OR(95%CI)分别为1.591(1.286~1.969)、2.364(1.758~3.179)、1.396(1.120~1.740)、1.227(0.999~1.634);汉族和朝鲜族合并人群的超重、整体型肥胖和分别以WHR和WHtR作为判断标准的腹型肥胖与高血压患病呈正相关,其OR(95%CI)分别为1.680(1.444~1.955)、2.77(2.230~3.457)、1.268(1.096~1.466)和1.263(1.069~1.492),且均随着体质指数水平的增高,高血压患病危险性增高。结论不同类型肥胖指标均与高血压患病密切相关,周身型肥胖合并不同类型腹型肥胖均使高血压患病危险升高。

Abstract:Objective To understand the relationship between indices of different types of obesity and the hypertension among the Korean-nationality and Han-nationality in Yanbian rural area. Methods With simple random sampling and the cluster sampling, a total of 5 838 subjects aged 20-70 years (including 2 852 of Korean nationality with age of 53.4±10.8 yrs, 2 986 of Han nationality with age of 47.5±11.4 yrs) were selected form each 2 villages of 3 townships of Yanbian rural area during August 2006 to August 2008, and their height, weight, waist circumference, hip circumference and blood pressure were measured. Results The prevalence rates of hypertension and grade 1 to 3 hypertension in the different types of obesity groups were obviously higher than that in the normal group (P〈0.05). The prevalence of hypertension was obviously increased in the group with both abnormal BMI and indexes of central obesity than in group with only one abnormal index of obesity (P〈0.01). After adjusted by age, sex, education, smoking and drinking, the logistic regression analysis showed that the prevalence of hypertension was positively associated with overweight (OR=t.736, 95%CI: 1.400-2.152), general obesity(3.320, 2.373-4.647)and central obesity diagnosed by WHtR ( 1.425,1.161-1.749)in Korean and positively associated with overweight (OR=1.591, 95%CI: 1.286-1.969), general obesity (2.364,1.758-3.t79) and central obesity diagnosed by WC (1.396, 1.120-1.740) and WHR (1.227, 0.999-1.634) in Han. The prevalence of hypertension was positively associated with overweight, general obesity and central obesity diagnosed by WHR and WHtR in both of Korean and Han, the value of OR (95%CI) was 1.680 (1.444-1.955), 2.77 (2.230-3.457), 1.268 (1.096- 1.466) and 1.263 (1.069-1.492), respectively. Conclusion Different types of obesity were closely related to the prevalence of hypertension, general obesity combined with different types of central obesity possibly increased the risk of hypertension.
