摘 要: | 目的观察CT血管成像(CTA)评价胰腺癌血管侵犯的特点,探讨血管侵犯与手术切除的相关性。方法选择2011年9月到2013年11月收治的胰腺癌患者120例,采用256层CT进行影像学观察和CTA分析,评价血管侵犯情况与手术可切除情况。结果 120例患者CTA检查血管侵犯78例,血管未侵犯42例,血管侵犯率为65.0%,其判断胰腺癌血管受侵情况与手术探查结果(63.3%)差异无统计学意义(P>0.05)。CTA检查评价为不可切除者67例,可切除53例,可切除率为55.8%,其判断胰腺癌可切除情况与手术探查结果(54.2%)差异无统计学意义(P>0.05)。结论CTA能有效确定胰腺癌侵犯血管的具体部位和程度,提高胰腺癌可切除准确率,值得推广应用。

Abstract: | Objective To observe CT angiography (CTA) 's evaluation of vascular invasion charac- teristics of pancreatic cancer, and discuss the relation between vascular invasion and surgical resection. Methods Selected 120 cases of pancreatic cancer patients from September 2011 to November 2013. Used the Philips 256 slice CT imaging for CTA analysis and evaluated the vascular invasion and tumors" resect- ability. Results CTA found that 78 cases had vascular invasion in the 120 patients, 42 cases non-vascular invasion, thus the vascular invasion was 65.0%. While surgical exploration found that the vascular invasion rate was 63.3%. There were no significant differences between the two methods ( P 〉 0. 05 ). CTA found that 67 patients were unresectable, 53 patients resectable, thus the resectable rate was 55.8%. While sur- gical exploration found the resectable rate was 54. 2%. There were no significant differences between the two methods ( P 〉 O. 05 ). Conclusion CTA can effectively determine the exact location and extent of vascular invasion of the pancreatic cancer and improve the accuracy of respectability that should be applied. |
