线粒体乙醛脱氢酶基因突变型对人心肌缺血再灌注损伤的保护作用

摘    要:目的:观察人线粒体乙醛脱氢酶2(ALDH2)基因突变型对法洛四联症患者体外循环术后心肌缺血再灌注损伤的保护作用。方法:入选61例行法洛四联症根治手术的患者,采用等位基因特异性PCR法分析线粒体乙醛脱氢酶基因型,根据基因型不同分为乙醛脱氢酶2基因野生型(野生型)组(n=38)和乙醛脱氢酶2基因突变型(突变型)组(n=23)。两组分别于体外循环升主动脉开放后20 h由颈内静脉取血,测定心肌肌钙蛋白水平。记录两组体外循环时间、主动脉阻断时间、术后24 h变力性药物评分≥15的例数和术后住院天数≥14 d的例数。结果:两组患者性别、年龄、体重、术前经皮氧饱和度、室间隔缺损大小、主动脉骑跨率、体外循环时间和主动脉阻断时间比较差异无统计学意义(P〉0.05)。野生型组术后20 h肌钙蛋白值显著高于突变型组(P=0.018)。野生型组术后24 h变力性药物评分≥15的比率显著大于突变型组(P=0.028)。野生型组术后住院天数≥14 d的比率显著大于突变型组(P=0.048)。结论:线粒体乙醛脱氢酶基因突变型对心肌缺血再灌注损伤具有保护作用。
