全髋关节置换术后不同时间开始康复训练的效果研究

目的研究3种不同的开始时间对全髋关节置换(THA)术后患者实施康复的影响,选择出康复效果最佳的时间。方法选择本院THA术后患者90例,用抽签法随机分为3组(各30例)。分别于术后第1天(THA1组)、第3天(THA2组)、第10天(THA3组)开始按统一制定的康复训练计划实施功能锻练,并于术后第1、3、6个月末采用Harris髋关节评分法进行组间比较。结果术后1个月末THA1与THA3组、THA2与THA3组Harris评分差异有显著性意义(P<0.01);术后3个月末THA1与THA2、3组的Harris评分差异均有显著性意义(P<0.01);术后6个月末3组间Harris评分差异无显著性意义。结论在保证生命体征平稳的前提下,康复训练于术后第1天开始为最佳;THA术后早期(1~3d)开始功能锻炼效果优于晚期(10d)开始锻炼,但术后1d或3d开始锻炼可视患者具体情况而定。

Rehabilitation effect of training in different periods after total hip joint replacement

Objective To choose the best training time of patients undergoing total hip joint replacement.Methods Ninety patients underwent total hip joint replacement were randomly divided into 3 groups. Rehabilitation training was performed from the first day in group 1, the third day in group 2 and the tenth day in group 3 after operation. Harris score was used to evaluate the hip joint functions of the patients in three groups respectively in 1 month, 3 months and 6 months after operation.Results The score of group 1 and group 2 had significant difference to group 3 by the end of 1 month (P<0.01); the score of group 1 was obviously higher than group 2 and 3 by the end of 3 months (P<0.01); Six month later, there was no differences among three groups.Conclusion To begin rehabilitation training in 1~3days is more effective than in 10 days after operation. The rehabilitation training should begin from the first day after operation if the patients' vital signs are all right.
