VEEG长程监测对癫痫的诊断及致痫灶定位的意义

目的:探讨录像脑电图(VEEG)长程监测对癫痫的诊断及致痫灶定位的意义。方法:对50例癫痫患者分别进行常规脑电图(REEG)检查和 VEEG 长程监测,比较两种监测方法。结果:REEG 检查50例患者中,痫样波检出率38%,定位率32%。经 VEEG 长程监测痫样波检出率78%,定则定位33例,定位率66%。VEEG 长程监测与 REEG 相比,特异性波检出率及定位准确率前者明显高于后者,统计学上有显著性差异(P<0.05)。结论:VEEG 长程监测明显优于 REEG,与其他检查如 CT/MRI、SPECT、PET 等结合运用,可进一步提高癫痫的诊断率及定位准确性,对难治性癫痫的外科治疗有着重要意义。

SIGNIFICANCE OF LONG- TERM VEEG MONITORING IN THE DIAGNOSIS OF EPILEPSY AND THE LOCALIZATION OF EPILEPTOGENIC FOCUS

Objective:To explore the significance of long-term VEEG monitoring in the diagnosis of epilepsy and the localization of epileptogenic focus before surgical operations.Method:REEG and long-term VEEG monitoring were examined separately in 50 patients with epilepsy and the two methods were compared.Re- sults:After examination of REEG,the rate detected of epileptiform discharges was 38%,the rate of localization was 32%;After examination of long-term VEEG monitoring,the rate detected of epileptiform discharges was 78%.Accurate lateralization and localization in 33 patients were determined,the rate of localization was 66%. Comparing long-term VEEG monitoring with REEG,the rate detceted of specificwave and accuracy for localization the former was significantly higher than the latter.There was significant difference in statistics(P<0.05).Con- clusions:Long-term VEEG monitoring might have significant advantages over REEG.Combination with other de- terminations such as CT/MRI,SPECT and PET,it might further improve the accuracy for the diagnosis of epilep- sy and the localization of epileptogenic focus and it might have important significance for the surgical treatment of intractable epilepsy.
