髂嵴高度与腰椎失稳性疾病的相关性研究

目的探讨髂嵴高度与腰椎失稳性疾病的关系,为腰椎失稳性疾病的进化医学研究提供依据。方法选取2017年9月至2018年6月于北京积水潭医院就诊的腰痛患者40例、腰椎间盘突出症(LDH)患者40例、腰椎滑脱患者40例和腰椎退行性侧弯(DS)患者40例为观察组,健康志愿者40例为对照组。通过X线像测量各组研究对象的髂嵴绝对高度和L4椎体高度,计算髂嵴相对高度(髂嵴绝对高度/L4椎体高度)并进行组间比较。同时对各组研究对象的年龄和性别构成进行比较。应用SPSS 19.0统计软件对数据进行处理。结果 LDH组和腰椎滑脱组的髂嵴绝对高度较低,但与其他各组的差异无统计学意义;DS组的髂嵴绝对高度和髂嵴相对高度均为各组中最低值,与对照组及腰痛组的差异均有统计学意义;DS组的平均年龄高于其他各组,差异有统计学意义。结论 DS的发病与年龄相关,且可能与患者髂嵴高度较小有关。

Relationship between the iliac crest height and lumbar unstable disease

Objective To investigate the relationship between the height of iliac crest and lumbar instability,and to provide evidence for evolutionary medical study of lumbar instability.Methods Forty cases of low back pain,40 cases of lumbar disc herniation(LDH),40 cases of lumbar spondylolisthesis and 40 cases of degenerative scoliosis(DS)were selected as observation groups,and 40 cases of healthy volunteers were selected as control group in Beijing Jishuitan Hospital between September 2017 and June 2018.The absolute height of iliac crest and the vertebral body height of L4 were measured by X-ray.The relative height of iliac crest(absolute height of iliac crest/vertebral body height of L4)was calculated.The height of the iliac crest,the relationship between the height of the iliac crest and the diseases,sexes and ages of all groups were compared and analyzed.SPSS 19.0 was used to process data.Results The absolute heights of iliac crest were lower in LDH group and lumbar spondylolisthesis group,but the differences were not statistically significant.Both of the absolute height and relative height of iliac crest in DS group were the lowest,and there were statistical differences between DS group and control group,low back pain group.Average age of DS group was older than other groups,and the deference was statistically significant.Conclusion DS is an age-related degenerative diseases and may be associated with lower iliac crest height.
