HBV阳性肝病者肾组织内HBSAg和HBCAg的形态研究

从尸检材料中获得３０例ＨＢＶ阳性的肝组织，观察其组织学，并进行ＨＢＳＡｇ、ＨＢＣＡｇ的免疫组化染色。根据ＨＢＡｇ的染色形态，将其分成两类：反映病毒活跃复制的第一类（１８例）；病毒可能呈整合状态的第二类（１２例）。同时检测肾组织内ＨＢＳＡｇ和ＨＢＣＡｇ的表达情况，其ＨＢＡｇ阳性者１２例，阳性率为４０．０％。其中肾小球阳性者１０例，分布主要在系膜区和沿毛细血管袢，也有核内ＨＢＣＡｇ表达；肾小管ＨＢＡｇ阳性１２例，其中ＨＢＳＡｇ阳性１１例，形态有膜型、弥漫型、周边型和包涵体样型，ＨＢＣＡｇ阳性５例，形态有弥漫型、膜型或伴间质分布及核内表达。肾小管ＨＢＡｇ表达与肾小管和肾间质病变有关。

Study on the Patterns of HB SAg and HB CAg in Renal Tissue of Patients with HBV Positive Hepatitis

The hepatic tissue from autopsy of 30 HBV positive cases and were examined with immunohistological staining of HBsAg,HBcAg.According to the pattern of HBAg,it was divided into 2 categories,the first category(18 cases)related to active replication of HBV DNA and the second category(12 cases)probably reflect the integration of HBV DNA to host genome.30 cases were examined at the same time for the HBsAg and HBcAg expression of renal tissue.Of them,12 cases were HBAg positive,with the positive rate 40.0%,10 cases with glomeruli HBAg positive,distributed mainly in the mesangial areas and along capillary loops,and some had nuclear expression.There were 12 cases with positive HBAg of ranal tubules, of which 11 were positive HBsAg,including membranous,diffuse,marginal and clusion body like patterns.5 cases with positive HBcAg,whose patterns were ofcytoplasmic,membranous or with interstitial and intranuclear patterns.The expression of HBAg of renal tubule was related to renal tubule and interstitial pathological changes.
