洁净手术室医院感染管理效果测评

目的 加强洁净手术室医院感染管理。方法 健全管理组织,建立管理制度,包括工作人员和病人管理,室内用物管理,清洁消毒管理。加强重点监测,分3个阶段对3个洁净手术室空气、物体表面、工作人员手进行采样,并重视管理效果。结果3个阶段空气。物体表面、工作人员手卫生学检测,其菌落数分别经方差分析两两比较,方差分析结果,空气菌落数F=19.8,P<0.05,有显著性差异,物体表面和工作人员手无显著性差异。3个阶段空气菌落数两两比较均为P<0.05,有显著性差异,第3阶段空气菌落数明显减少。3个阶段检测总合格率经两两比较P均为<0.05,有显著性差异,说明第3阶段管理效果优于前2个阶段。结论 要使洁净手术室发挥预期的效果,必须投入管理力量,否则即使设施先进,也未必能产生良好的效果。

Evaluation on effects of hospital cross infection control in clean operation rooms by laminar flow

Objective To enhance the management of hospital cross infection in clean operation rooms by laminar flow. Methods The completion of administrative organizations and establishment of administrative regulations were carried out to enhance the management over the faculties and in-patients, in-door stuff, cleaning and disinfection. Key monitoring was done by sampling the air in the operation room, the surface of objects and the hands of medical workers for testing at 3 stages. Then hygienic tests were conducted in view of the air, the surface of objects and the hands at the 3 stages. The number of colony was compared with Chi-square test and paired comparison. Results Chi-square test showed significant difference in colony in the air (F=19.8, P<0.05), but insignificant as to the surface of subjects and the hands of medical workers. Paired comparison showed there existed significant differences not only in the numbers of colony in the air between the 3 stages ( q=3.0, q=8.6 and q=5.1, P<0.05, P<0.05, P<0.05 respectively), but also in the surfaces of objects between the 1st stage and 3rd stage (q =3.2, P<0.05) but no significant differences between the 1st and the 2nd stages as well as the 2nd and 3rd stages; In view of the colonies at the hands, no differences were found to be significant between the 3 stages. The qualification rates at the 3 stages for all items were Chi-square tested, showing x2=12.0, P<0.05; X2= 118.9, P<0.05; x2=51.2, P<0.05 respectively. Conclusion The completion of administrative organizations, establishment of administrative regulations and enhancement of key monitoring are a promising means in putting clean operation rooms by laminar flow into action more effectively.
