p16基因在大肠癌中的表达及临床意义

探讨抑癌基因p16在大肠癌中的表达及其表达与肿瘤的病理组织学分级、临床分期、淋巴结转移和预后的关系。方法　采用免疫组化SP方法标记 70例大肠癌组织中抑癌基因p16。结果　 70例大肠癌中p16基因表达的阳性率为 6 4 3 % ,在高、中、低分化大肠癌中p16基因阳性率分别为 84 2 %、5 3 2 %和 2 9 4% ;在DukesA、B、C、D期大肠癌中p16蛋白阳性率分别为 90 0 %、81 8%、5 0 0 %及 37 5 %。在 38例伴淋巴结转移的病例p16蛋白的阳性率为 44 7% ,而 32例不伴淋巴结转移病例p16蛋白阳性率为 87 5 % ;生存期 3年以上组p16蛋白阳性率为 84 1% ,显著高于生存期 3年以下组的 38 1% (P <0 0 5 )。结论　p16基因的表达缺失与大肠癌的发生有关 ,而且p16基因与大肠癌的病理组织学分级、临床分期、淋巴结转移和预后有关。p16基因的检测可作为大肠癌生物学行为和预后评价的参考指标。

Expression of p16 Gene in Colorectal Cancer and Its Clinical Significance

Objective To investigate the expression of tumor suppress gene p16 in human colorectal carcinoma and its relation with pathological gradings,clinical stagings,lymph node metastasis and prognosis of tumor.Methods Tunnor suppress gene p16 in 70 cases of human colorectal carcinoma were labeled by S-P immunohistochemical technique.Results The positive rate of p16 expression in 70 cases were 64.3%.Iu high,mediate and low differentiated colorectal carcinomas,the rate was 84.2%,53.2% and 29.4%,respectively.In Dukes A,B,C,D,the rate was 90.0%,81.8%,50.0% and 37.5%,respectively.In 38 cases with and without lymph node metastesis the rate was 44.7% and 87.5% respectively.In the group of the survival times of over 3 years and of ander that period the rate was 84.1% and 38.1% respectively( P <0 05).Conclusions The inactivation of p16 gene expression is related to the colorectal cancer occurance.The p16 gene is related with pathological gradings,clinical stagings,lymph node metastasis and prognosis of colorectal cancer,and p16 gene detection can be considered as a prediction of malignant potential and prognosis.
