老年腹膜透析患者反复发作腹膜炎的原因及护理

目的探讨老年腹膜透析患者反复发作腹膜炎的原因及护理措施。方法选择近3年内，复旦大学附属华东医院肾内科接受CAPD治疗并反复发生腹膜炎的老年患者14例，通过面谈、查阅腹膜透析手册记录、电话随访等进行资料收集分析原因，并提出相应的护理对策。结果老年人、自身免疫力低下、预防知识的欠缺、营养不良、无菌操作意识不强、手卫生不合格是反复发作腹膜炎的高危因素。结论加强对患者及其照顾者的操作培训和再培训，强调使之定期化、制度化有助于降低老年腹透患者腹膜炎的发病率，提高其生存质量及生存率。

Nursing measures for recurrent peritonitis in elderly patients with end stage renal failure and treated with continu-ous ambulatory peritoneal dialysis

Objective To investigate the causes and nursing measures of recurrent peritonitis in elderly patients with end stage renal failure and treated with continuous ambulatory peritoneal dialysis （CAPD）. Methods Fourteen old uremic patients who had been treated with CAPD in the Department of Nephrology of Huadong hospital and had recurrent peritonitis in the past 3 years were enrolled in this study. We tried to find the causes of recurrent peritonitis for these patients by in- terview, checking the recording books, and follow up by telephone. Nursing measures were suggested based on the data mentioned above. Results The most common causes of recurrent peritonitis in this patients were old age, low immunity, poor knowledge of preventing infection, malnutrition, lack of strong consciousness of aseptic processing, and disqualification of the hands. Conclusions Periodic training of the manipulative skill of dialysis procedure for patients and their close relatives was very important for the prevention of peritonitis as well as improvement of life quality in the elderly patients.
