维生素D对2型糖尿病患者血糖控制及炎性反应的影响

摘    要:
目的探讨维生素D(VD)对2型糖尿病(T2MD)患者的血糖控制、代谢综合征及炎性标志物表达的影响。方法选择该院就诊的T2MD患者93例,按照随机数字表法,分为观察组45例,对照组48例。观察组患者采用额外补充VD,对照组患者使用相同计量的安慰剂。治疗3、6个月后,观察2组患者血糖控制及炎性标志物表达的变化。结果补充VD治疗3、6个月后,观察组患者25(OH)D和1,25(OH)2D3均升高,与对照组比较,差异有统计学意义(P0.05)。治疗3个月后,2组患者空腹血糖(FBG)、餐后2h血糖、糖化血红蛋白(HbA1c)均有一定程度的下降,且观察组下降程度大于对照组;2组患者空腹胰岛素水平有所升高且观察组高于对照组。6个月之后,FBG、餐后2h血糖、空腹胰岛素水平及HbA1c,2组比较差异有统计学意义(P0.05)。与对照组患者比较,观察组治疗后Ca2+水平提高,但差异无统计学意义(P=0.071)。治疗6个月后C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)均有所下降,CRP和IL-62组间比较,差异有统计学意义(P0.05)。结论补充VD能够改善T2MD患者血糖控制及炎性反应。

Effect of Vitamin D on glycemic control and inflammatory reaction of type 2 diabetes mellitus
Abstract:
Objective To explore the Effect of Vitamin D on glycemic control ,metabolic syndrome and inflam‐matory reaction of type 2 diabetes mellitus (T2MD) .Methods 93 cases of T2MD meeting the criterion were chosen and randomly divided into treatment group (45 cases) and control group (48 cases) .Treatment group was given extra vitamin D supplements ,the control group was given corresponding placebo .After three/six months treatment ,the glycemic control and inflammatory reaction were analyzed .Results After three/six months treatment ,the level of 25 (OH)D and 1 ,25 (OH)2 D3 of treatment group were significantly higher than control (P < 0 .05) .Besides ,the lever of FBG ,2h postprandial blood glucose (PBG) ,and HbA1c had decreased after three months treatment ,the insulin level increased that the degrees of treatment group were higher than control ;After six months treatment ,the differ‐ences between treatment group and control were statistically significant (P < 0 .05) .Moreover ,Ca2 + level had in‐creased and there was no statistical difference (P = 0 .071) .Inflammatory factors of CRP ,IL‐6 and TNF‐α had de‐creased after treatment ,the differences were statistically significant (P< 0 .05) .Conclusion Vitamin D supplementa‐tion could improve the glycemic control and inflammatory reaction of type 2 diabetes mellitus .
